Did you know that scar tissue can continue to affect the body long after an injury or surgery has healed?

Restrictions from scars may contribute to tightness, discomfort, reduced mobility, altered movement patterns and sometimes pain in completely different areas of the body.

One of my favourite parts of treatment is helping clients improve the appearance, mobility and function of scar tissue through targeted bodywork techniques. These before and after photos show just how much change can be achieved (shared with permission). Client was 93yrs old and the hip replacement scar was “30 or 40yrs old”, they’d been unable to bend or straighten leg in that time and required fluid tablets to help with build up and retention. After 2 sessions a month apart in the April and May, client was able to bend, weight bare, no longer required fluid tablets and had less pain. Further pictures show the body continuing to heal without treatment over the following months.

𝗧𝗵𝗲 𝗧𝗿𝘂𝘁𝗵 𝗔𝗯𝗼𝘂𝘁 𝗦𝗽𝗶𝗻𝗮𝗹 𝗜𝗻𝗷𝗲𝗰𝘁𝗶𝗼𝗻𝘀: 𝗔𝗿𝗲 𝗪𝗲 𝗣𝗮𝘆𝗶𝗻𝗴 𝗳𝗼𝗿 𝗣𝗹𝗮𝗰𝗲𝗯𝗼𝘀?

▪️ Chronic non-cancer spine pain is a massive global health challenge that carries a significant socioeconomic burden.

▪️ To manage this condition, particularly in North America, patients are frequently offered interventional procedures such as epidural steroid injections, facet joint nerve blocks, and radiofrequency ablation.

▪️ However, these procedures are expensive, not curative, and subject to heavily conflicting clinical guidelines.

▪️ A rigorous systematic review and network meta-analysis detailed in the sources evaluated just how effective these common interventions really are.

▪️ By analyzing 81 randomized controlled trials involving 7,977 patients, the sources deliver a sobering conclusion:

𝗧𝗵𝗲 𝗠𝗮𝗶𝗻 𝗙𝗶𝗻𝗱𝗶𝗻𝗴

▪️ Commonly performed interventional procedures for chronic spine pain provide little to no pain relief or improvement in physical functioning compared to sham (placebo) procedures.

𝗧𝗵𝗲 𝗜𝗹𝗹𝘂𝘀𝗶𝗼𝗻 𝗼𝗳 𝗘𝗳𝗳𝗶𝗰𝗮𝗰𝘆: 𝗧𝗵𝗲 𝗦𝗵𝗮𝗺 𝗘𝗳𝗳𝗲𝗰𝘁

▪️ Invasive procedures are known to carry large "non-specific" or placebo effects, meaning patients often feel better simply because they underwent a highly medicalized procedure.

▪️ To find the true specific effects of these interventions, the researchers rigorously compared them against sham procedures.

▪️ The review separated chronic spine pain into two distinct categories to assess treatment effectiveness:

𝗔𝘅𝗶𝗮𝗹 𝗦𝗽𝗶𝗻𝗲 𝗣𝗮𝗶𝗻

▪️ Pain localized to the spine itself.

▪️ For this type of pain, there is moderate certainty evidence that epidural injections of local anaesthetic (with or without steroids) and joint-targeted steroid injections provide little to no difference in pain relief compared to a sham procedure.

▪️ Other treatments, like epidural steroid injections or joint-targeted injections of local anaesthetic, also showed little to no difference, though the evidence is of lower certainty.

▪️ Alarmingly, low certainty evidence suggests that intramuscular injections of local anaesthetics combined with steroids might actually increase pain severity.

𝗥𝗮𝗱𝗶𝗰𝘂𝗹𝗮𝗿 𝗦𝗽𝗶𝗻𝗲 𝗣𝗮𝗶𝗻

▪️ Pain radiating from the spine along a nerve root (such as sciatica).

▪️ For radicular pain, moderate certainty evidence shows that epidural injections of local anaesthetic and steroids, as well as radiofrequency ablation of the dorsal root ganglion, probably result in little to no difference in pain relief.

▪️ Epidural injections of just steroids or just local anaesthetics similarly showed little to no difference, based on low certainty evidence.

𝗣𝗵𝘆𝘀𝗶𝗰𝗮𝗹 𝗙𝘂𝗻𝗰𝘁𝗶𝗼𝗻𝗶𝗻𝗴 𝗮𝗻𝗱 𝗛𝗮𝗿𝗺𝘀

▪️ Beyond simple pain relief, patients generally seek these procedures to improve their daily physical functioning.

▪️ Unfortunately, the review found that interventional procedures probably result in little to no difference in physical functioning for both axial and radicular pain.

𝗔𝗱𝘃𝗲𝗿𝘀𝗲 𝗘𝘃𝗲𝗻𝘁𝘀

▪️ Furthermore, these procedures are not without physical risks.

▪️ Low certainty evidence indicates that some interventions—such as joint radiofrequency nerve ablation, joint-targeted injections of local anaesthetics and steroids, and intramuscular injections—may actually increase the risk of non-serious adverse events when compared to sham procedures.

𝗪𝗵𝘆 𝗧𝗵𝗶𝘀 𝗠𝗮𝘁𝘁𝗲𝗿𝘀 𝗳𝗼𝗿 𝗣𝗮𝘁𝗶𝗲𝗻𝘁 𝗖𝗮𝗿𝗲

▪️ Currently, medical guidelines conflict: some organizations recommend against invasive treatments for low back disorders, while others strongly endorse all commonly performed interventional procedures.

▪️ The review detailed in the sources aims to settle this controversy by providing a highly rigorous baseline of evidence.

▪️ The findings heavily suggest that many patients are undergoing expensive, invasive, and potentially harmful treatments that offer no more clinical benefit than a placebo.

✅ 𝗧𝗵𝗲 𝗧𝗮𝗸𝗲𝗮𝘄𝗮𝘆

▪️ According to the comprehensive data in the sources, no commonly performed interventional procedure offers convincing evidence of important pain relief or physical functioning improvement for chronic spine pain.

Don’t know how I found time to fit this in, but so glad I did 🕰️

So sorry, but there won’t be any Day One class today… I thought I broke my ankle/foot but after x-rays, it looks like it’s just ligament damage 😭 off to the physio for more assessments and treatment this afternoon.
