Instead of assuming breathing difficulty → record respiratory rate, oxygen level, and physical signs you observe.
Instead of labeling anxiety → describe behaviors and include the patient’s own words.
Instead of saying confusion → note orientation, responses, and need for redirection.
Instead of saying fatigue → document level of alertness, activity, and patient statements.
📌 If you didn’t see it, hear it, measure it, or quote it — don’t chart it.

✔ strengthen clinical communication
✔ stay aligned with professional standards

You’re asked to chart for a task you didn’t actually do or to “document later” because the shift is chaotic and everyone is trying to catch up.
It may seem like teamwork — but falsifying or back-dating documentation is illegal and can cost you your license.

It may feel right to help — but in nursing, good intentions do not override the law.
Working outside your scope, skipping documentation, or accepting unsafe assignments can put your license at risk — even when you’re trying to support patients or colleagues.
