My patient wanted to improve support through the mid-face while keeping the result subtle and in keeping with her natural features.

Treatment was carried out to both the medial and lateral cheeks, to restore gentle support through the central cheek area, while improving definition and contour along the cheekbone.

The aim was not to dramatically change the shape of the face, but to create a fresher and more balanced appearance.

As we age, changes in facial fat, bone structure and skin quality can all contribute to changes through the mid and lower face. Dermal filler can be helpful where targeted volume and structural support are needed.

During consultation, we also discussed other treatments, including Sculptra, and how a combined treatment approach may help to support the face more comprehensively over time. Different treatments work in different ways, so combining them can be useful when looking at volume, structure, skin quality and collagen stimulation.

Managing expectations is an important part of treatment planning...

During consultation with my patient, we discussed that the anticipated outcome may require more than one treatment.

The nose is a highly vascular anatomical area, and treatment requires careful consideration of vascular anatomy, tissue characteristics, treatment plane, product placement and the volume of dermal filler being injected.

In areas such as the nasal bridge, there is limited soft-tissue, meaning a conservative approach to treatment is particularly important.

For this reason, it is sometimes not appropriate or possible to achieve the full desired outcome in a single appointment (or at all, with dermal filler alone).

Injecting more filler than is clinically appropriate can compromise the aesthetic outcome and increase the risk risk of migration or vascular complications. Careful assessment, technique and conservative treatment planning are extremely important.

In this case, a first treatment was planned, with the understanding that the result would be reviewed once everything had settled and the tissues had recovered. If clinically appropriate, a second appointment can then be considered to further refine and optimise the result.

Every nose (and face) is different. Treatment should always be individualised according to the patient's anatomy, clinical assessment and treatment goals.

Good aesthetic medicine isn't only about knowing when to treat, but also knowing when to stop.

Dermal filler was carefully placed above the dorsal hump to create a smoother transition along the nasal dorsum and disguise its appearance, with a very subtle lift to the nasal tip.

The aim was not to change the character of the nose, but to make subtle adjustments to improve overall balance and harmony while maintaining a natural result.

Initial swelling and redness are present in the immediate post-treatment image and this will settle as the area heals.

As with any aesthetic treatment, results vary between individuals and treatment suitability is assessed on an individual basis following consultation.

Nose filler maintenance.
My patient visited the clinic 15 months ago for profile balancing, and has now returned for a touch up after noticing her dorsal hump beginning to reappear, and a slight reduction in tip projection. 

She could have waited a little longer before having maintenance treatment, however, with a holiday coming up, she wanted to refresh her result beforehand.

Following assessment, a very small amount of Stylage XXL dermal filler was used to disguise the hump and restore a subtle lift to the nasal tip. 

As with all aesthetic treatments, the timing of maintenance appointments varies from patient to patient. Factors such as individual anatomy, metabolism, previous treatment and the type of product used can all influence how long a result is maintained.

Treatment is always tailored to the individual following a full consultation and suitability assessment.

Choosing the right type of dermal filler is just as important as the treatment technique itself.

These are my patient’s lips one year after treatment.

This is a good example of why filler choice is an important part of achieving not only the desired initial result, but also a result that continues to look natural as the filler gradually integrates and breaks down over time.

For this treatment, I used Teosyal RHA, which is specifically designed to move, stretch, and bend naturally with dynamic facial expressions.
The lips are a highly mobile area, so choosing a product with appropriate rheological properties is an important consideration when aiming for softness, natural movement and longevity.

Dermal filler does not simply disappear overnight. It is gradually broken down by the body over time, and the rate at which this happens varies between individuals. Product choice, treatment technique, placement, metabolism and the movement of the area can all influence how a result evolves.

This is why I believe it is important not to focus solely on how lips look immediately after treatment, but also on how the chosen product and treatment approach allow the result to settle and age over time.

When considering facial aesthetics, it is important to look beyond individual features and consider how each area relates to the rest of the face.

For this lovely patient, we treated the cheeks, chin and nose using Stylage XL, and lips using Stylage Special Lips, with the aim of creating greater harmony and improving overall facial balance.

The cheeks were treated to provide structure and subtle definition to the mid-face, with treatment focused predominantly on the right side to address the asymmetry in volume and definition compared to the left.

The chin was carefully treated to add subtle forward projection but also vertical length to elongate the lower face, helping to improve overall facial proportions and profile balance.

The patient had previously visited the clinic two years ago for nose filler, and in the recent treatment there was still a visible result from this. As some of the indentation above and below the dorsal hump was beginning to reappear, only a very small amount of dermal filler was required to maintain results.

The lips were also treated with consideration given to their natural proportions and their relationship with the rest of the face.

This is why I believe a detailed facial assessment is so important. Treating several areas as part of one cohesive plan allows subtle changes to work together, rather than treating each feature in isolation.
Every treatment plan is individual and based on the patient's anatomy, previous treatment and desired outcome.

Facial balancing is about treating the face as a whole rather than focusing on a single area.

In this case, filler was placed within the medial cheeks (apples) to restore support and soften the transition through the mid-face, the lateral cheeks to provide subtle definition of the cheekbone, and the chin to improve lower facial projection and overall facial proportions.

Treating these areas together, where appropriate, creates a more harmonious relationship between the mid and lower face while maintaining natural facial balance.

The patient had previously had chin treatment, so only a small amount of dermal filler was required to maintain results.

Lip filler is not just about creating volume. Careful placement can improve hydration, definition and balance, whilst respecting each patient's natural anatomy.

These before and after images show a subtle refinement designed to maintain a soft, natural appearance.
