As aesthetic practitioners, we’re not simply injecting into “skin” or “lips” - we’re working in an area containing an intricate network of arteries, veins and vessels, many of which have important connections and variations from person to person.

Before depositing product, aspiration can be used as an additional safety precaution. In this case, blood was drawn back into the needle/syringe, indicating that the needle tip may have been within a blood vessel.

Had product been deposited while the needle was within a vessel, there is a risk of a vascular occlusion (VO).

A vascular occlusion can occur when dermal filler enters a blood vessel or compresses/compromises a vessel, disrupting blood flow to the tissues supplied by that vessel. This can result in ischaemia, tissue damage and, in severe cases, necrosis. And an important point: aspiration is an additional precaution — not a guarantee of safety. A negative aspiration does not mean that a vessel has definitely not been encountered.

This is exactly why anatomical knowledge, injection technique, patient assessment and complication management are such fundamental parts of aesthetic practice. Understanding where the vessels are, how they travel, the potential anatomical variations and what to do if something isn’t right is what allows us to make informed, safety-focused decisions throughout a treatment.
