Dehiscence is a condition where there is a localized loss of alveolar bone covering the root surface of a tooth, exposing a portion of the root beneath the gums. Unlike normal bone support, the bone over the root becomes very thin or completely absent in a specific area.

Several factors can contribute to dehiscence, including:
• Thin alveolar bone
• Prominent or malpositioned teeth

This video shows severe and unusual tooth enamel loss, a condition caused by long-term abrasive oral hygiene practices. During history taking, the patient explained that 30–40 years ago, toothbrushes and toothpaste were not commonly available in many villages. People often used sand, charcoal, ash, and other rough materials to clean their teeth.
Over time, these abrasive methods gradually wore away the enamel surface, leading to:

Many people ignore cavities when they look small from outside. But once decay crosses the enamel and reaches the dentine, the spread becomes much faster. Dentine is softer than enamel, allowing bacteria to spread sideways beneath the surface.

In an undermined cavity, the outer enamel may appear small or partially intact, while underneath there is a large hidden destruction of dentine.
⚠️ This means:
• The visible hole may look small
• But internally, the tooth may be extensively damaged
• The enamel can suddenly collapse due to lack of support

If neglected:
• Decay rapidly progresses toward the pulp

I often talk about oral diseases because I strongly believe that people take oral health seriously only when they understand the consequences of neglect. Many dental problems become severe simply because treatment is delayed until it is too late.

🦷 Neglected Crown Fracture: From Small Injury to Cyst Formation

This picture shows a fracture involving nearly half of the tooth crown, extending deep enough to expose the pulp tissue. Initially, the tooth was painful for a few days, but once the pain disappeared, the patient ignored it for the next 4 years.

🔬 What Happened Inside the Tooth?
When the pulp is exposed after trauma or fracture, bacteria enter the tooth and gradually destroy the pulp tissue. Over time, the pulp becomes necrotic (dead).
Because the pain often stops after pulp death, many patients mistakenly believe the tooth has healed. In reality, the infection silently spreads beyond the root tip.

⚠️ Clinical & Radiographic Findings
On examination, the tooth was non-vital, and radiographic evaluation revealed a cystic lesion associated with the root. What started as a simple crown fracture had progressed into a chronic periapical pathology, making treatment far more difficult and invasive.

🛠️ Why Early Treatment Matters
If treated early, the tooth might have required:
✔️ Simple restoration
✔️ Root canal treatment
But delayed treatment increased the complexity and may now require:
• Surgical intervention
• Extensive endodontic treatment
• Possible extraction in severe cases
