ACUTE CONJUNCTIVITIS
Bacterial conjunctivitis is a highly contagious eye infection caused by bacteria. It can spread quickly but is easily managed with the right care.
⚠️ The Warning Signs
* **Redness & Swelling:** Bloodshot appearance in one or both eyes.
* **The “Sticky” Factor:** Thick, yellow, or greenish discharge.
* **Morning Crusting:** Eyelids glued shut upon waking up.

🚫 How to Prevent It
Prevention is all about breaking the chain of transmission.

⚠️ CONJUNCTIVITIS aka PINK EYES ‼️
A young female presented in OPD with complaints of redness, itching and photophobia associated with watery discharge
O/E : Follicles in both Palpebral Conjunctiva
Dx : Viral Conjunctivitis (common these days in our region)
Viral conjunctivitis is a highly contagious infection of the eye, commonly caused by adenoviruses. It usually presents with redness, watering, irritation, burning sensation, and sometimes swelling of the eyelids. One eye is often affected first, followed by the other.
Good hand hygiene, avoiding eye rubbing, and not sharing towels or cosmetics are important to prevent spread. Most cases improve on their own within 1–2 weeks with supportive care such as cold compresses and lubricating eye drops.
⚠️ Seek ophthalmic evaluation if there is severe pain, decreased vision, or sensitivity to light.

⚠️ ANTERIOR STAPHYLOMA ‼️
Anterior staphyloma is a severe ocular condition characterized by the ectasia (bulging) and thinning of the anterior fibrous coat of the eye, lined posteriorly by incarcerated uveal tissue.
🔍 Key Clinical Features:
• The “Bluish-Black” Appearance: Marked thinning of the cornea/limbus allows the dark pigment of the underlying iris/uvea to show through, giving it a characteristic slate-blue color.
• Etiology: It typically arises as a late complication of a perforated corneal ulcer, severe ocular trauma, or uncontrolled absolute glaucoma.
• Pathophysiology: The structural integrity of the cornea is lost, leading to iris incarceration. Over time, intraocular pressure causes this combined, weakened cicatricial tissue to protrude forward.
• Management: Depending on the visual potential, structural integrity, and presence of pain, management ranges from tectonic penetrating keratoplasty to enucleation/evisceration for cosmesis and pain relief.

A 7 year old boy presented in Ophthalmic OPD with history of trauma in right eye
VA - Perception Of Light
IOP - 30
Total hyphema is a condition in which the anterior chamber of the eye becomes completely filled with blood, usually after trauma. The eye may appear dark or black, causing severe reduction in vision and eye pain.
Common Causes
* Blunt eye trauma
* Post-operative bleeding
* Bleeding disorders
* Neovascularization of the iris
Symptoms
* Sudden vision loss
* Eye pain and redness
* Photophobia
* Raised intraocular pressure
Complications
* Corneal blood staining
* Secondary glaucoma
* Optic nerve damage
* Rebleeding
Treatment
* Bed rest with head elevation
* Eye shield protection
* Cycloplegics and topical steroids
* Control of intraocular pressure
* Surgical washout if persistent or complicated

Central Corneal opacity After Foreign Body
Dissociated Vertical Deviation (DVD)
🔹 What is it?
A form of strabismus where one eye drifts upward spontaneously when binocular vision is disrupted (e.g., covering one eye).
🔹 Key Features
• Slow upward drift with extorsion
• No corresponding downward movement of the fellow eye (dissociated)
• Often bilateral but asymmetric
• Associated with latent nystagmus
🔹 Common Associations
• Infantile esotropia
• Poor binocular fusion
• Early-onset strabismus
🔹 Pathophysiology
Thought to be due to abnormal supranuclear control of eye movements and defective fusion mechanisms.
🔹 Clinical Significance
• Becomes evident on cover test
• Can affect cosmesis more than vision
• May coexist with inferior oblique overaction
🔹 Management
• Observation if mild
• Surgical correction (e.g., superior re**us recession) in significant cases
