Past and current medical and surgical history as well as family history
Drug and food allergies information updates on an ongoing basis
Medication list of all prescription and non-prescription (over-the-counter) currently taking (dosages included)
DIAGNOSIS
Use the information obtained in the assessments about a patient to identify risks, make tentative, differential, and specific diagnoses, determine appropriate treatment options and plans, and identify the cause(s) of the patient’s condition(s) at the highest level of specificity possible. Readily provide information and advice to patients about care and treatment, such as goals and expectations, the procedures involved, the potential adverse effects, the anticipated costs, the available providers, as well as, advice or counseling on the cause of their condition(s), systemic conditions evidently affecting their appearance. Also readily provide information and advice to the patient about the options for treatment, the prognosis with treatment, the prognosis without treatment, the urgency of the situation, and preventative measures.
