A midweek quiet win from a Tuesday follow up. Nothing to do with the scale.

'I walked up the stairs at my office yesterday afternoon carrying my laptop bag and I did not stop to breathe on the landing!'

She is six months in, on a GLP-1 with hormone support. The stair landing without needing to stop is the thing she celebrated. Not the pounds. Not the jeans. The functional life improvement!

We collect these sentences on purpose. They are the truest signal that the body underneath is actually different, not just smaller.

'My hemoglobin is normal, so my iron is fine.'

We hear this in almost every perimenopause and midlife consult, and it is one of the most common misreads on a lab printout. Here is the short version.

Hemoglobin tells us if you are anemic today. Ferritin tells us what your iron stores actually look like.

You can have a hemoglobin of 13.4 and a ferritin of 14. Not anemic on paper. Functionally empty in real life. That gap shows up as cold hands, 3am anxiety, breathlessness on stairs you used to run up, hair thinning at the temples, and fatigue that eight hours of sleep does not touch.

What to ask for at your next visit: ferritin, iron, TIBC, transferrin saturation. All four. Read together.

If your fatigue has not budged and your provider says 'your labs are normal,' that is the follow up question. Save this for the visit.

The full iron panel we ask for in a perimenopause or midlife metabolic workup. Save this to your perimenopause weight loss or lab checklist board.

1. Ferritin. The real iron stores number.
2. Serum iron.
3. TIBC, total iron binding capacity.
4. Transferrin saturation.
5. Hemoglobin and hematocrit, for context.

Hemoglobin alone will miss functionally low iron in midlife women almost every time. All five together tell the story.

If your fatigue has not budged, ask for the full panel at your fall follow up.

Monday check-in

What is the ONE micro-goal you are picking for this week? Not the whole fall reset. One - Little - Thing.

In clinic we ask new patients to name ONE doable adjustment for the next seven days, out loud, before they leave the consult. Not five. One. Because the person who names one and does it in week one is the person who is still doing five by week eight. The person who names five in week one is almost always the person who is doing none by week three.

It works way more often than it should… and once I’m there, I almost always end up doing more than I planned.

Ten 15 to 20g protein snacks for perimenopause, GLP-1, and back-to-routine weeks. Save this to your protein cheat sheet board and use it when a full meal feels like too much.

1. 3/4 cup cottage cheese with berries. 20g.
2. 6oz plain Greek yogurt with a spoon of chia. 17-20g.
3. Two hard boiled eggs and a string cheese. 20-25g.
4. A single serve tuna packet on 4 crackers. 17g.
5. Turkey roll ups, 3oz deli turkey with a wedge of cheese. 18g.
6. Edamame, 1 cup shelled, warm and salted. 12-15g.
7. A scoop of whey in cold milk, kept in the fridge for 3pm. 30g.
8. Two boiled shrimp skewers from the deli. 18g.
9. A protein bar you already trust, aim for 15g+ and under 5g added sugar.
10. Rotisserie chicken pulled from the fridge, small palm, 18g.

'I wake at 3am and lie there until 5, and I do not know if it is hormones or stress or coffee.'

We hear this in almost every perimenopause consult, and we heard it four times this past week alone. Here is how we actually read the 3am wake up in clinic, so you can read it too.

If you fall asleep fine but wake at 3am with your heart pounding, that is often a cortisol spike on top of a progesterone dip. Progesterone is calming, and it drops first in perimenopause.

If you fall asleep hard and wake at 3am wide awake but calm, look at blood sugar. A low glucose dip in the second half of the night wakes the brain. Protein at dinner and a small pre bed snack often fixes it inside a week.

If you wake at 3am hot, that is usually estrogen fluctuation. HRT can quiet it, and it is one of the fastest things to shift on treatment.

One pattern, three mechanisms. We work through which one is yours in a first consult. Save this for your next visit, ours or anyone else's.
