I know that a lot of you are having to stay home more with all that is going on around us and coming to an in-person essential oil class may be a challenge. I don’t want any of you to miss out on learning about alternative safe and effective options for yourself and your families.

TRIANGULAR INTERVAL SYNDROME (TIS)

[ANATOMY LESSON]

The Triangular Interval (also known as the lateral triangular space, lower triangular space and triceps hiatus) is a space connecting the axilla to the posterior compartment of the arm. It is one of the three intermuscular spaces found in the axillary space. The other two spaces are Quadrangular Space and Triangular Space.

Triangular Interval Syndrome (TIS) was described as a differential diagnosis for radicular pain in the upper extremity.
It is a condition where the radial nerve is entrapped in the triangular interval resulting in upper extremity radicular pain.

The radial nerve and profunda brachii pass through the triangular interval and are hence vulnerable. The triangular interval has a potential for compromise secondary alterations in thickness of the teres major and triceps. It is described based on cadaveric studies that fibrous bands were commonly present between the teres major and triceps. When these bands were present, rotation of the shoulder caused a reduction in cross sectional area of the space.

Hypertrophy of the teres major can occur secondary to weight training and potentially compromise the triangular interval with resultant entrapment of the radial nerve.

Shoulder dysfunctions have a potential for shortening and hypertrophy of the teres major.

The triceps brachii has a potential to entrap the radial nerve in the triangular interval secondary to hypertrophy. The presence of a fibrous arch in the long head and lateral head further complicates the situation.

ELBOW, FOREARM OR HAND PAIN?

EXTENSOR CARPI RADIALIS LONGUS MUSCLE (ECRL)

The ECRL originates proximally to the distal 1/3 of the lateral supracondylar ridge of the humerus and inserts at the base of the 2nd metacarpal bone.

Therefore, it does not attach to the lateral epicondyle and is NOT part of the “tennis elbow” / lateral elbow tendinopathy group that attaches via the common extensor tendon (belly tendon) group.

It is superficial for its entire course except where it runs deep to the two deep compartment posterior forearm muscles of the thumb: abductor pollicis longus and extensor pollicis brevis.

Note: the ECRL crosses the wrist joint posteriorly, so it extends the wrist (as its name implies), but it is part of the “radial group” of muscles (along with brachioradialis and extensor carpi radialis brevis), that it crosses the elbow joint anteriorly, so it flexes the elbow joint!

Its main function is a radial deviation of the hand, which means it “kinks” your wrist in way that your thumb is getting moved towards the inner side of your forearm and your radius, respectively.
Furthermore, together with the other extensors of your wrist, it extends and stabilizes the latter during gripping motions and this way prevents it from flexion.

If your ECRL is too tight or contains tender and/or trigger points (TPs), it can give you pain at the lateral side of your elbow.
Additionally, you can experience pain all the way down your forearm and to the backside of your hand (Pic 4). Another common symptom is a weak and painful grip, which even can make you drop light things (a glass of water).

The most common cause of the development of tight muscle tissue and TPs in the ECRL is excessive use of a strong grip.
A strong grip is often needed in various sports and activities of daily living.
This is why, among others, the following movements and/or activities can overload this muscle:
▪️Tennis (especially back hand play)
▪️Rock climbing
▪️Weight lifting (heavy dead lifts)
▪️Gardening
▪️Hard screw driving

SHOULDER OR ARM PAIN?

TERES MINOR MUSCLE

[ANATOMY, FUNCTION & PATHOLOGY]

Teres minor is a narrow muscle which lies below infraspinatus, above teres major and triceps brachii, and deep to deltoid. It is one of the four muscles which comprise the Rotator Cuff.

It originates at the upper two-thirds of the lateral border of the scapula.
The upper fibres end in a tendon which inserts into the inferior facet of the greater tubercle of the humerus. The lower fibres insert into the humerus directly below the inferior facet of the greater tubercle of the humerus.

Teres minor, along with Infraspinatus, primarily produces external rotation of the shoulder joint.
It assists in adduction and extension of the shoulder.
When the humerus is stabilized, it abducts the inferior angle of the scapula.

In concert with the other rotator cuff muscles, Teres Minor is instrumental in providing stability to the shoulder joint, and helps to hold the humeral head in the glenoid cavity of the scapula.

Muscular problems in the teres minor muscle can mimic and feel like bursitis in your shoulder.

Common people that suffer are the ones that also are prone to suffer from infraspinatus problems. Those include, but are not limited to Rock climbers, Tennis/Volleyball/Handball/Baseball players, Swimmers or Painters.

Trigger points in your teres minor can make it ache right at the location of these spots, but also send pain to other, more distant parts of your body.
If your teres minor contains trigger points, it mainly will give you pain at the side of your shoulder (Pic 3).
Beside that, pain can radiate slightly into the backside of your arm.
Because of this, it can contribute to back of shoulder pain!

As this muscle supports the infraspinatus you will understand that trigger points or excessive muscle tension in this muscle will affect the same movements. (Check my previous posts for more information about the infraspinatus muscle!)

But in general, the movements that will cause pain or that may be impaired are the ones where you rotate your shoulder outwards and reach backwards, like putting on your jacket.
