Pain during s*x can have many possible causes and it can affect both women and men.

Sometimes the discomfort is related to hormones, pelvic-floor function, inflammation, infection, s*xual health conditions, or other urologic or gynecologic concerns.

Understanding where the pain happens, when it happens, and what other symptoms occur alongside it can help your provider determine what may be causing it.

For women, painful s*x — also called dyspareunia — may be associated with va**nal dryness, hormonal changes, genitourinary syndrome of menopause (GSM), vulvodynia, pelvic-floor dysfunction, or recurrent infections.

The cause is not always obvious, which is why persistent or recurring pain deserves a medical evaluation.

Men can experience painful s*x, too.

Possible causes may include Peyronie’s disease, prostatitis, chronic pelvic pain syndrome, pelvic-floor dysfunction, inflammation, infection, or pain associated with erections or ej*******on.

Talking openly with a urologist can help identify what is contributing to the discomfort and what treatment options may be appropriate.

When evaluating painful s*x, how the pain presents matters.

Is it near the opening or deeper in the pelvis? Does it happen during erections, pe*******on, or ej*******on? Is it new, recurring, or getting worse?

Those details can provide important clues about the underlying cause.

S*x should not routinely hurt.

UTIs after menopause are common, but they shouldn’t be ignored.

As estrogen levels decline, va**nal and urinary tissues can become thinner, drier, and more easily irritated. These changes can make some patients more prone to recurrent UTIs, burning, urgency, or discomfort.

Low-dose va**nal estrogen may help support the va**nal and urinary tissues and reduce recurrent UTI risk when prescribed appropriately.

If you’re dealing with repeated UTIs after menopause, it may be time to ask what’s causing them, not just treat the next infection.

Is va**nal estrogen cream safe to use?

For many patients, yes, when it is prescribed and monitored by a clinician.

Vaginal estrogen cream is a low-dose, localized treatment. That means it is applied directly to the va**nal tissue to help with symptoms related to low estrogen, such as:
• Vaginal dryness
• Burning or irritation
• Pain with s*x
• Recurrent UTIs
• Urinary urgency or discomfort

This is different from systemic hormone therapy because the goal is not to raise estrogen throughout the whole body. The goal is to support the va**nal and urinary tissues that can become thinner, drier, and more irritated after menopause.

That said, it may not be right for everyone. If you have a history of estrogen-sensitive cancer, unexplained va**nal bleeding, or other medical concerns, talk with your healthcare provider before using it.
