There is no such thing as "hair loss" as one condition. There are multiple biological processes people call hair loss - and the LOCATION of the change is often the strongest clue to which mechanism is happening.
Follicles are not uniform across the scalp. The top expresses far more androgen receptors than the sides and back, which is why androgenetic patterns form where they do. Immune-driven patterns can appear almost anywhere because every follicle has its own immune privilege system. Hairline edges and temples carry the highest mechanical load, while diffuse shedding across the entire scalp usually reflects a systemic shift rather than a localized follicle problem. Scarring conditions like frontal fibrosing alopecia have their own highly recognizable signatures.
This is why pattern recognition matters. In trichology, the location of the loss often narrows the differential to two or three conditions before a single test is run. Trichologists are trained to read these patterns - and to distinguish what looks similar from what is actually the same.
The reason many people feel like "nothing works" is often because the mechanism being treated does not match the pattern present.
