Using my face to talk about trans periods with you!!
TW: Menstruation, dysphoria, maneuvering the health system as a trans person
As long as I’ve had my period, I’ve been dealing with debilitating cramps. Language and treatment around mensuration is so gendered that for a long time I avoided trying to moderate or understand the pain. When the pain would get so bad that I didn’t have a choice, I was often met by resistance, even frustration from OB/GYNs and ER doctors & nurses who couldn’t understand why I wouldn’t just use birth control to moderate it. They had no understanding as to how this solution might trigger my dysphoria (let alone what that is) so I continually left appointments with no answers, no solutions.
It was only last year when I finally got the nerve to go back to an OB/GYN, determined to find out what was really going on as the pain worsened and persisted. I was able to find a trans-competent OB/GYN, and mere seconds into examining me, this doctor knew exactly what was happening. Because of so many years of extreme cramps, my pelvic floor has adopted spasms and gotten itself into the mess that is called Pelvic Floor Dysfunction Disorder. I had literally never heard of this before. And because I was told over and over again it was “just a bad period” or “maybe endometriosis, but we can’t really know”, it got worse than it ever should have. Thankfully, I now have a better understanding of what is happening to my body and have been able to begin the journey of healing and therapy to help release this knot of muscle that is my pelvic floor!
All of this is to say:
We MUST stop using gendered language when talking about periods. We MUST include trans folx in the conversation. Trans folx MUST be able to walk into a doctors office or an emergency room and be treated without question or a laugh or a look of confusion on the face of the person who is supposed to have all the answers. We MUST NOT feel like the better option is to endure pain and cause more trauma to our bodies, instead of being taken care of and healed. My fellow white transmasc folx and I MUST recognize our privilege in our experience of the health system.
🍄 All humans have a pelvic floor, including humans with a p***s!
🍄 When I first started treating this population, my patients were all older men post prostate surgery due to prostate cancer
🍄 Luckily, I have a network of really great urologists who are familiar with pelvic floor muscle dysfunction of all kinds
🍄 Currently, 90% of my patients in this population are men between the ages of 20 and 45 with tight pelvic floors not just weak ones (psst it’s much more common than you might think, it’s just that nobody talks about it)
🍄 What are some typical symptoms of pelvic floor muscle dysfunction with these patients? Pain with p*eing, pain with s*x (during/after), erectile dysfunction, p*eing all the time, groin and back pain, pain with moving your bowels, feeling like you’re not fully emptying when you use the bathroom, post p*e dribbling (common, not normal)

👩🏼⚕️As a pelvic floor physical therapist, I am asked this a lot.
🤷♀️With so many brands and gadgets out there, what’s the right choice for you?
🍓My answer: It depends! (I know most people want a definitive answer here, but it’s more complex)
🆘 Kegels may not be right for you. As we discussed in the last post, you may have pelvic floor muscle tightness in which case kegels will make your symptoms worse and therefore are not healthy to do. Second, you may not be doing them properly. You may not be relaxing the muscles after which is also an important component, and you may be compensating with other muscles thereby making them much less effective etc.
🗝 So the real question is: 𝗦𝗵𝗼𝘂𝗹𝗱 𝘆𝗼𝘂 𝗯𝗲 𝗞𝗲𝗴𝗲𝗹𝗶𝗻𝗴? If the answer is yes, and you’ve been assessed by a pelvic floor PT to make sure you’re doing them properly, then go for it! Ask your PT for guidelines with reps/sets.
✔️ These gadgets can be very helpful. They’re motivating. They make an exercise which most women (and men-yes men can Kegel too) dread, less of a bummer!

What the Trump administration did Friday was despicable. They reversed the regulations prohibiting discrimination in healthcare against patients who are transgender. Also THIS week, two black trans women were killed. Enough is enough. * I think my sign says it all, but as a physical therapist who specializes in pelvic health, s*xual health, pain, dysfunction etc., know I’m out here, I’m horrified, disgusted, and I treat all pronouns.

As I say all the time “Baby tweaks lead to MAMA changes” Whatever the goal, start small but stay consistent. Major changes and commitments can be overwhelming right now. You’d be surprised how effective starting small can be.
Start with 3 mins of meditation instead of 10, exercise for 15 mins not a full hour, eliminate one kind of irritant/trigger food vs 5, drink 1/4 glass less of wine... You will work up to all of it, but for now take the first step! then, congratulate yourself! 🤩You did it 👌👏👏👏
