"The stress hormone" is the label everyone knows cortisol by, and it's the least interesting part of what it actually does. Swipe through for how it works across your whole day, and why the number on a results page depends entirely on when it was drawn. β°

PCOS gets talked about in soundbites and myths, often before anyone gets a straight explanation of what it actually is. Dr Chris Giezing breaks down what it is, what tends to show up in a woman's biomarker panel, and one diagnosis myth worth clearing up.

Every iTHRIVE journey starts in the same place: a full biomarker panel, not a quick chat about how you're feeling. Dr Chris Giezing explains why that starting point matters, and what it can pick up that a conversation alone never will.

Most guidelines don't give you an age. They give you a symptom list.βοΈ
Testosterone starts a slow, steady decline from around age 30, about 1% a year. By your mid-40s, that adds up to a meaningful drop, but almost no man has a number from his 30s to measure it against.
Here's what the actual clinical guidelines say: test when there are symptoms, low energy, low libido, mood changes, loss of muscle, or specific risk factors like unexplained anaemia, bone density loss, or diabetes.
That approach is built to catch a deficiency. It was never built to track a trend.
If the first test you ever take is the one triggered by symptoms, you have nothing from before to compare it to. The decline isn't measured. It's assumed.
And even then, a real diagnosis isn't one number. Guidelines require two separate morning tests, on different days, plus symptoms alongside the result, before anything is confirmed.
Waiting for symptoms means comparing yourself to a population range built to flag disease, not to your own baseline. Those are two different questions.
Knowing where you stood at 32 makes 45 mean something. Without that starting point, you're not tracking change. You're guessing at it.
Get your baseline before you need it.

Your biomarker results came back normal. So why don't you feel normal?
That result is one reading, taken on one day. It landed inside the normal range, so it looked unremarkable on paper. But your body's natural systems during this stage of life do not decline in a steady, predictable line. They can swing significantly from day to day, sometimes even within the same day.
Your test happened to land in the normal-looking window. On a different day that same week, it could have landed somewhere else entirely on that same fluctuating line.
This is part of why women in their 40s experiencing these changes are sometimes told everything looks fine, when what they are actually experiencing is unpredictable variation that a single test was never designed to capture. One snapshot does not tell the full story. A pattern does.
Knowing where your wellness actually stands takes more than one reading on one day.

Your body does not malfunction one symptom at a time. It works as a system, and it declines as one too.
The wellness tree is how iTHRIVE maps that system. The roots and trunk are everything happening beneath the surface: your biomarkers, your sleep, your nutrition, your stress load. The parts you cannot see but that determine everything above them. The branches and canopy are what you feel and what others notice. Your energy. Your sharpness. How you recover. How you show up.
When the canopy starts thinning, the answer is never in the canopy itself. It is always in the root system.

Patients tell us the same thing, often word for word: same food, same training, different body. In most cases, the honest answer isn't diet. It's estrogen.
Three signs worth paying attention to.
βοΈ Your shape has changed more than your weight. As estrogen declines, fat shifts from the hips and thighs toward the abdomen, even when the scale hasn't moved.
βοΈ It isn't happening alone. If your midsection is changing alongside irregular cycles, hot flushes, or disrupted sleep, that's one cause, not four.
βοΈ The approach that used to work has stopped working. That's because the fat itself has changed type, from just under the skin to the kind packed around your organs, and that type behaves differently.
This matters beyond appearance. Visceral fat is linked to insulin resistance and real cardiovascular risk, which is why I track waist size in patients, not just weight.
None of this is a failure of discipline. It's estrogen changing where your body stores fat, and that's measurable, not something to guess at.

πͺ Your skin isn't separate from the rest of your biology. It's often the first place internal biomarkers show up, sometimes before you feel anything else.
This one's worth watching properly: real signs on the skin that can point to deficiencies most people never think to check for.
iTHRIVE's clinical team reads those signals properly, not guesswork. Take the wellness test and find out what your skin might already be telling you. Link in bio.
