SS-31 — In 2004, a compound built to relieve pain turned out to do something else entirely: concentrate itself almost exclusively inside mitochondria, the part of the cell that generates energy. Pharmacologist Hazel Szeto and chemist Peter Schiller were testing synthetic opioids when they found it — a peptide accumulating inside mitochondria at levels roughly five thousand times higher than the surrounding cell. They named it SS-31; its clinical name is elamipretide, sold under the brand name FORZINITY by Stealth BioTherapeutics. The clearest proving ground for it was Barth syndrome, a genetic disorder that leaves heart and skeletal muscle chronically short on energy from birth. The first trial, a 12-patient Phase 2/3 study, missed its primary endpoints — a six-minute walk test and a standard symptom scale — after 12 weeks. In the open-label extension that followed, at 36 weeks, patients improved by 95.9 meters on the walk test and 2.1 points on the symptom scale. A larger follow-up trial, TAZPOWER, tracked 180 patients over 168 weeks and recorded a cumulative 96.1-meter improvement, along with measurable gains in knee extensor muscle strength — the specific endpoint the FDA cited when it granted accelerated approval in September 2025. This approval carries a specific condition: confirmatory trials are still required to keep elamipretide on the market long-term. What Szeto and Schiller found by accident became the basis for a drug that targets the exact cells that fail first when mitochondria do: heart muscle, skeletal muscle.
