As a doctor, here’s an unpopular opinion...

We aren’t always the best people to look after your skin.

It might sound strange coming from someone with access to the most powerful treatments, the strongest prescriptions and the ability to burn away almost any concern. But that isn’t skincare, is it? And we need to be more hoenst about where our expertise actually lies.

Yes, there are things that should always remain medical. Anything injectable, anything carrying a risk of vascular occlusion or anaphylaxis, lasers, prescription medicines. That’s our territory, and rightly so. But so much of the work that should come before any of that is about the health of the skin itself, its barrier, its condition, its foundation. And when it comes to that daily, hands-on care, the most experienced people usually aren’t the doctors.

It’s the skin therapists. The ones doing this every single day, learning constantly from the sheer volume of skin they treat, refining what works and discarding what doesn’t. They provide something genuinely invaluable, and often a real sanctuary too, calming and restorative in a way that shouldn’t be underestimated.

The truth is, optimising skin health often resolves half the concerns before aggressive treatment is even needed, and it’s skin therapists doing that work.

So let’s champion collaboration over hierarchy. We can all learn from each other, and when it comes to daily skin care, skin therapists get my vote every time! 08/09/2026

This is exactly why I only recommend prescription tretinoin for max 6 weeks. Before you "treat" the problem, make sure the skin has a healthy barrier function first.🌷
