Your skin might not be “purging”—it might be inflamed.

I see this often when prescription tretinoin and azelaic acid are introduced together for acne.

Tretinoin speeds up skin-cell turnover, while azelaic acid helps target congestion and inflammation. Both can be useful, but both can also cause irritation—especially when the skin is sensitive or the barrier is already impaired.

Watch for:
• Burning or stinging
• Persistent redness or heat
• Tight, shiny or flaky skin
• Moisturiser suddenly stinging
• Increased sensitivity
• Breakouts looking angrier and taking longer to heal
These aren’t signs that you need to “push through” or that the prescription is working harder.

When your barrier becomes impaired, it loses water more easily and becomes less effective at protecting your skin from irritants. This can create more inflammation—and inflamed skin is much harder to clear.

Your barrier is the foundation of healthy skin. If it isn’t functioning properly, continuing to push strong ingredients can keep you stuck in a cycle of irritation, sensitivity and breakouts.

You don’t need to suffer through angry skin to achieve results.

Four weeks between these photos.

When this client first came to me, her skin was inflamed, reactive and struggling after using tretinoin. In an attempt to repair the dryness and irritation, she had also been applying heavy barrier creams—which had left her skin feeling increasingly congested.

At this stage, her skin didn’t need another strong active or aggressive acne treatment. It needed us to take a step back.

We simplified her routine, changed her home care to Synergie Skin and completed a series of LED treatments to help calm the inflammation and support the repair process.

In just four weeks, you can already see significantly less redness, fewer active breakouts and a much calmer overall skin response.

We’re not at the end of her journey, but we now have healthier, more resilient skin to work with—and that has to come before we start focusing on congestion, marks or scarring.

More products don’t always mean better skin.

One of the most common things I see in clinic is an overwhelmed routine—multiple serums, overlapping active ingredients, frequent exfoliation and treatments being added before the skin has had time to respond.

When the skin is inflamed, reactive or breaking out, adding more can make it increasingly difficult to identify what is helping and what is contributing to the problem.

My approach is usually to simplify first.

Remove what the skin doesn’t need. Support the barrier. Choose ingredients with a clear purpose. Introduce change gradually and assess how the skin responds.

The same applies to treatments. Your skin doesn’t need to be constantly pushed to make progress. It needs the right treatment, at the right time, with enough recovery in between.

A considered routine may look simple—but every step should be doing something valuable.

Less, but better.

Some of the products marketed as “gentle” or “barrier repairing” aren’t necessarily the right fit for an acne-prone skin.

That doesn’t mean every ingredient on this list is bad—or that it will break everyone out. It means we need to look at the complete formula, where the ingredient sits on the list and what your skin is actually doing while using it.

A rich recovery balm may be helpful after irritation but too heavy as an everyday moisturiser. A cleanser may be labelled gentle but still leave your skin tight, reactive or overproducing oil.

If your skin isn’t improving, check the products you use every day before adding another active.

Save this post, check your labels and send it to someone whose “sensitive skin” routine may need a second look.

Ingredient lists and formulations can change. Persistent acne, burning, itching or dermatitis should be assessed individually.

One month between these photos.

So far, we’ve made a few targeted changes to her homecare and completed one lymphatic treatment—and look at the reduction in inflammation.

There is still some minor active acne and scarring to work through, but before we move into scar revision, we need the skin calm, supported and functioning well.
Trying to treat scarring while the skin is still inflamed can mean more irritation and slower healing. This first stage has been about reducing that inflammation, supporting the barrier and creating healthier skin to work with.

We’re not at the end result yet, but this is a very good place to start. I can't wait for this transformation in a few months!
