Trying to show what happens in the body. A model that is testible and researchable, that should cover most of the them out there.

Problem - what do we call it?

Subluxation is full of baggage and weird definitions.
Fixation is just not the whole picture.
VCS covers only the spine and needs some updating.

Neuro-Biomechanical Complex - that's what it is, but think we can do better on the name.

Hands On or Hands Off - Exercise vs. Manual Therapy + Exercise: What Works Best for Back Pain? 25/02/2026

Exercise vs. Manual Therapy + Exercise: What Works Best for Back Pain? (2025 Review)

Description:
We know exercise is the "gold standard" for chronic low back pain, but does adding hands-on treatment actually make a difference? ๐Ÿ‹๏ธโ€โ™‚๏ธ๐Ÿ‘

In this video, we break down a brand new 2025 Systematic Review published in the Journal of Bodywork and Movement Therapies.

Researchers analyzed 10 Randomized Controlled Trials (RCTs) to answer one specific question: Does adding manual therapy (manipulation, massage, etc.) to an exercise program improve pain and disability more than doing exercise alone?

The results challenge the "exercise only" approach, showing clear benefits to a combined strategy.

In this video, you will learn:
โœ… The 80% Success Rate: How 8 out of the 10 included studies found that adding manual therapy resulted in significantly better improvements in pain or disability compared to exercise alone.
โœ… Short-Term Wins: Why the review concludes that manual therapy is highly effective for short-term relief, which may help patients adhere to their long-term exercise programs.
โœ… Modalities Compared: The specific effects of different hands-on treatments, including Spinal Manipulation, Myofascial Release (MFR), Massage, and Muscle Energy Technique (MET).
โœ… The "Thoracic" Mistake: Why 2 of the studies failed to show a benefitโ€”and how the researchers link this to the use of thoracic (mid-back) manipulation for low back pain instead of lumbar adjustments.
โœ… Neurophysiological Mechanisms: How manual therapy stimulates muscle spindles and inhibits pain pathways (Gate Control Theory), potentially "priming" the nervous system for movement.
โœ… Radiculopathy Relief: One study highlighted where adding massage specifically improved symptoms of nerve pain and paraesthesia (pins and needles).

The Mid Back Crack Evidence - Does it Help Neck & Shoulder Pain? 25/02/2026

We often adjust the thoracic spine (mid-back) to help with neck and shoulder issues, but what does the latest high-level evidence actually say? ๐Ÿ“‰๐Ÿค”
In this video, we review a brand new 2025 Systematic Review published in the Journal of Manipulative and Physiological Therapeutics. Researchers analyzed 26 studies to determine the effectiveness of Thoracic High-Velocity Low-Amplitude Thrust (Tx-HVLAT) manipulation for "Upper Quadrant" disordersโ€”including neck pain, headaches, and shoulder issues.
The results highlight the quality of evidence currently available and how thoracic adjustments stack up against sham treatments, mobilization, and exercise.

In this video, you will learn:
โœ… The Scope: The review looked at 5 specific conditions: Neck pain, Cervical radiculopathy, Cervicogenic headache, Temporomandibular disorders (TMD), and Shoulder pain.
โœ… The "Quality" Reality Check: Why researchers concluded there is "very low to low-quality evidence" that thoracic manipulation is more effective than mobilization, deep neck flexor training, or sham treatments for neck and headache issues.
โœ… The Shoulder Pain Nuance: How thoracic manipulation was found to be more effective than "general conservative therapy" for shoulder pain... BUT not more effective than sham manipulation. โœ… The Comparison: How this study used the GRADE method to compare adjustments against other common therapies like cervical manipulation and thoracic mobilization.
โœ… The Takeaway: Understanding why statistical heterogeneity and "Risk of Bias" make it difficult to declare a clear winner in this area of manual therapy.
