Your voice is a direct expression of who you are inside and out.
Your larynx (voice box) is fascially connected to your heart and respiratory diaphragm. The extrinsic muscles around the larynx are fascially connected to the endothoracic fascia, which connects to the respiratory diaphragm, transversalis fascia and the fascial pelvic diaphragm. I have just had the pleasure of dissecting this connection on a fresh tissue cadaver. It was amazing!

What does this mean- tension in the throat will affect breath and adaptability in the pelvic diaphragm, and vice versa. Tension in the pelvic diaphragm will affect your voice.

Well, actually, I spent two evenings narrating a livestream dissection while watching the amazing Todd Garcia uncover the anatomy of the breath on a fresh tissue cadaver. It’s a conversation-stopper!

That description pales in comparison to what I experienced. This is my third time narrating a dissection, and it is one of the most mind-blowing. At one point, Todd asked me if I had anything to add, and I was speechless, emotional and close to tears at this stunning anatomy.

Friday Anatomy Fact: The Origami of the Deep Cervical Fascia

I haven't done a Friday Fact in a while and was inspired by a discussion I had the other day in a mentoring session. The deep cervical fascia has been a rabbit hole I have loved diving into, and it helps you understand the bigger picture, think of the musculature and the fascial connections as myofascial postcodes. So we aren't just talking about SCM and trapezius we are also talking about the fascial layer that surrounds these muscles.

The deep cervical fascia or fascia Colli in older textbooks lies under platysma and superficial cervical fascia and invests the neck muscles. This awesome origami of fascia is organised into layers supporting the structures of the neck. It is divided into the investing/ superficial layer, the pretracheal/ middle layer and the prevertebral/ deep layer. The first layer and the most superficial layer of the deep cervical fascia is the investing layer.

The Investing layer wraps around the neck, surrounding the deeper layers and splitting to encase both sternocleidomastoid and the trapezius. It is the most superficial layer of the deep cervical fascia. That's quite a mouth full, so to put it simply; it is the outer bag that wraps around another two bags of deep cervical fascia. Inferiorly it connects into the fascia of pec major.
How does it attach: Think of a tube with an attachment to the top, bottom, front and back.

Pretracheal layer or middle layer of deep cervical fascia:
Can be subdivided into visceral and muscular, the muscular layer invests the infra hyoids and two of the supra hyoids- mylohyoid and geniohyoid, the investing fascia invests the other two supra hyoids.

Clinical relevance: The exciting piece to me is that inferior, this layer of fascia connects into the clavipectoral fascia. The clavipectoral fascia wraps around pec minor and subclavius. Think about restrictions in pec minor and associated fascia and how that would translate up into the neck fascia.

The prevertebral layer of deep cervical fascia: The yellow line on the image
The prevertebral layer is the fascia that surrounds the cervical vertebra and the muscles associated with it. So you can see on the image it surrounds the posterior neck muscles and comes in front of the vertebrae to include the scalenes and the brachial plexus. It also forms the floor of the posterior triangle.

As you start to see how these layers of fascia intertwine, the final piece is how all 3 form the carotid sheath that contains the common carotid artery, internal jugular vein and vagus nerve.

I hope this shows how we can't just look at muscles - origins and attachments; we have to think of the fascial connections and the associated vascular structures.
