Damage can occur when extensions are outgrown and the weight of the extension shifts from the base of the natural lash, to the tip.

This is part of the reason we recommend refills every 2-3 weeks. We remove these lashes and place a fresh fan back to the base of the natural lash. Delaying refills and letting extensions grow out or fall off naturally is never a good idea as there is a chance of this damage occurring.
