CO₂ Laser – Eye Rejuvenation Consultation

What it involves:

For acne-scar treatment in Fitzpatrick Skin Type V, laser treatment can be effective, but the priority is to minimise post-inflammatory hyperpigmentation (PIH) and avoid unnecessary thermal injury.
Best laser options for Skin Type V
* PICO / PICO Hybrid – often a good starting option for pigmentation, acne-related discolouration and some textural improvement, with generally less downtime than ablative resurfacing.
* Fractional non-ablative laser – can improve mild-to-moderate atrophic acne scars while reducing the risk associated with aggressive resurfacing.
* RF microneedling – particularly useful for rolling and some boxcar scars. It can provide dermal remodelling without the same epidermal ablation associated with CO₂.
* Fractional CO₂ laser – can produce stronger scar improvement, particularly for deeper atrophic scars, but Skin Type V carries a substantially higher PIH risk. Conservative settings, appropriate patient selection and experienced treatment are essential.A sensible approach
For Skin Type V, I would generally consider a combination approach rather than immediately starting with aggressive CO₂:
Assessment → pigment control/preconditioning → RF microneedling or conservative non-ablative/PICO treatment → reassessment → consider fractional ablative treatment for selected deeper scars.
The scar type matters too:
* Rolling scars: RF microneedling ± subcision
* Boxcar scars: fractional resurfacing/RF microneedling
* Ice-pick scars: TCA CROSS or carefully selected focal techniques
* PIH/discolouration: PICO/Q-switched approaches and strict photoprotection
