Jowls develop due to a combination of anatomy, aging, and lifestyle factors — it’s not simply “loose skin.”
As we age, the mandible (jawbone) and midface bones gradually resorb. When this underlying bony support diminishes, the skin and fat that once sat firmly in place begin to shift downward, leading to fullness and heaviness along the jawline.

Facial fat doesn’t disappear — it redistributes:
Fat pads in the upper and midface lose volume
Fat pads in the lower face descend
This creates the familiar pattern of hollowness above and heaviness below, making jowls more noticeable.
From our mid-20s onward, collagen production declines by approximately 1% per year. Reduced collagen and elastin mean the skin loses its ability to rebound, allowing gravity to take over.

Muscle activity also plays a role. Certain muscles pull the face downward, including:
Platysma
Depressor anguli oris (DAO)
As lifting muscles weaken and downward-pulling muscles become more dominant, the lower face can appear to droop.
Genetics also influence how and when jowls develop. Factors include:

Heavier lower-face fat compartments
