Melanoma remains one of the most common cancers in Australia, but when detected early, outcomes are significantly improved.

Regular self-examination plays an important role in early detection. Patients are encouraged to become familiar with their skin and monitor for any new or changing lesions, particularly those that evolve in size, shape, or colour.

The ABCDE criteria (Asymmetry, Border irregularity, Colour variation, Diameter, and Evolving) provide a useful framework when assessing pigmented lesions. Any concerning features should be assessed by a qualified health professional.

Preventative measures are equally important. Consistent use of broad-spectrum sun protection, protective clothing, and avoidance of peak UV exposure can help reduce risk over time.

Melanoma awareness extends beyond March. Ongoing vigilance, regular skin checks, and early assessment remain key components of skin health.

Eyelid Surgery (Blepharoplasty) by Dr Assad Bangash. 2 weeks between photos. ⁠

〰️ A Blepharoplasty removes excess skin, fat & in some cases muscle from the upper & or lower eyelids. In the context of cosmetic blepharoplasty, the aim is to rejuvenate the eye or periorbital area. Incisions are placed in the natural skin folds of the lid, hiding surgical scarring. ⁠

〰️ Blepharoplasty (laser or surgical) is done under conscious sedation with blocks to the area. This ensures patient comfort during the procedure. Surgery takes 45-90 minutes to complete.⁠

〰️ How long is recovery?⁠

You will have bruising and swelling after your eyelid surgery. As a guide; ⁠

• Surgical only: Recovery in one week. Sutures out on days 5-6. ⁠
• Surgical & laser blepharoplasty: As above, but skin healing & redness for another few weeks. ⁠
• Laser blepharoplasty: Swelling for 4-5 days, redness for 4-8 weeks.⁠

The best results are seen at 1-2 months post-surgery, when swelling & redness subsides. ⁠

This case shows excision of a non-melanoma skin cancer on the back shoulder, followed by reconstruction using a modification of a Limberg (rhomboid) flap, often described as the “square-peg-in-a-round-hole” technique.

• Pre-operative marking:
The lesion is marked with planned surgical margins, along with a geometric flap design to allow controlled reconstruction after tumour removal.

• Excision:
The skin cancer is removed in full, creating a circular or oval defect.

• Reconstruction:
A modified Limberg flap is raised and transposed into the defect. Although classically rhomboid in design, this variation allows a geometric flap to be adapted to a rounded defect, redistributing tension and enabling closure without excessive distortion of surrounding tissue.

Why this technique is used:

• Preserves nearby skin and contour

• Distributes tension away from the central defect

• Reduces wound distortion compared with simple linear closure

• Optimises functional and cosmetic outcomes in skin cancer surgery

This type of flap is commonly used in areas where skin laxity is limited and thoughtful tension management is required.
