TCM Treatment for Acute Shoulder Bi Syndrome
In the landscape of Traditional Chinese Medicine (TCM), acute pain is often viewed as an obstruction of Qi and Blood. Nowhere is this more evident than in the acute onset of Shoulder Bi Syndrome (肩痹), a condition that can strike without warning, leaving the patient with a frozen, agonizing joint that defies movement. While chronic shoulder pain is common, the acute presentation—characterized by sudden, severe, sharp pain and complete immobility, this indicates requires immediate and decisive intervention.
This is where the dynamic synergy of Two Tan Localized Fire Cupping (拔火罐) and Two Tan Acupuncture for Bi Syndrome(针刺) offers a profound, almost miraculous, before-and-after effect. Why Two Tan Acupuncture and Fire Cupping ? It is because the invasion of "Cold-Damp"
In TCM, Bi Syndrome is fundamentally a blockage. The classic text, The Yellow Emperor's Inner Canon, states: "When Wind, Cold, and Dampness combine, they become Bi." For the shoulder, the acute emergency is typically driven by a sudden, aggressive invasion of Cold and Dampness (风寒湿邪). This often occurs when a patient is exposed to a draft while fatigued or after sweating, or during a sudden drop in temperature and wrong posture.
The pathology follows a clear trajectory: the Cold congeals the Blood, causing it to "stagnate." Dampness adds a heavy, viscous quality that obstructs the meridians. This leads to the hallmark of acute Bi: "Stagnant Qi and Blood" (气滞血瘀).
From a Yin-Yang perspective, the Cold pathogen is a Yin evil. It invades the Yang aspect of the upper body, suppressing the warming, moving function of the Yang Qi. The intense pain is the result of the Yang Qi attempting to battle the invading Yin Cold, leading to a "tension" or "constriction" that locks the joint. The treatment, therefore, must aim to warm the Yang and expel the Cold, restoring the balance.
The Path of Invasion by Cold and Dampness. When Cold attacks, it invades LI, SI and SJ channels, impeding the free flow of Qi.
Acupuncture points like LI-15 (Jianyu), SI-9 (Jianzhen), and TB-14 (Jianliao) are the "gates" of the shoulder. In the acute state, these points are often painful to pressure, indicating the blockage at these specific junctures. By acupuncture these points, we "open the gates" to allow the trapped Qi to escape. If the pain just started, acupressure can help.
