Forma Co-Owner | Medical Director

Behind every treatment at FORMA is a foundation of trust, safety, and expertise.

Beth, our Medical Director, is a Nurse Practitioner who has been practicing since 2018. Fun fact: she began her nursing career as an RN at Brooks Health Centre in 2013!

As Medical Director, Beth plays a vital role in overseeing our medical procedures and protocols, ensuring every treatment is safe, thoughtful, and grounded in evidence-based practice. While you may not see her face every time you walk through our doors, we’re proud to have her guiding FORMA every step of the way.

Outside of FORMA, Beth works as a Nurse Practitioner in fertility medicine and is a wife and mom to two busy girls - her favourite job of all✨

✨ MEET DOMINIQUE ✨

Forma Co-Owner | Aesthetic RN Injector

With nearly two decades of nursing experience and a passion for beauty and aesthetics, Dominique brings a unique blend of clinical expertise, precision, and an artistic eye to every treatment.

Dominique completed her Bachelor of Nursing in 2008 and spent 16 years working in emergency nursing before transitioning to Brooks Home Care. In 2021, she began her journey into medical aesthetics, combining her love of nursing with her passion for beauty and helping others feel confident in their own skin.

Committed to ongoing education, Dominique has completed advanced training with , Aesthetics Canada® - the botox course, Karin E, The Academy by Frida, Dentox Comprehensive Hands-On Training in Therapeutic & Aesthetic Applications of Botulinum Neurotoxins with Dr. Howard Katz, and hands-on training with Monica Sandhar RN.

Dominique believes aesthetic treatments should enhance your natural features, maintain balance, and help you feel like the best version of yourself. ✨

When she’s not at Forma, she’s busy raising her four beautiful children, who are her world. 🌎❤️
