An ACL tear doesn’t automatically mean surgery. It means the decision should be made on data, not default.
For athletes returning to cutting, pivoting and jumping sport, reconstruction remains the current standard of care. It restores passive stability and offers some protection against subsequent injury (de Jonge et al., 2024).
But for isolated tears, there’s no certain evidence reconstruction outperforms a well-designed non-operative program on osteoarthritis risk, knee function or return to activity (de Jonge et al., 2024). Isolated tears are the minority though. Roughly 74% of ACL injuries involve concurrent meniscal, ligament or cartilage damage (Arhos & Kvist, 2025).
What often decides the pathway is how your knee responds to load. Once range of motion, swelling and quadriceps symmetry are restored, structured neuromuscular and strength training allows you to be classified as a potential coper or a non-coper.
45% of athletes initially classified as non-copers became potential copers after ten sessions and potential copers had 2.7–2.9x the odds of two-year success, whether or not they had surgery (Thoma et al., 2019).
Athletes who remained non-copers did worse either way. That group needs a more intensive plan, not just a surgical date (Thoma et al., 2019).
And if surgery is the right call, preparation still decides the outcome. Progressive pre- and post-operative rehabilitation produced normative-range scores on 86–94% of KOOS subscales at two years, versus 51–76% under usual care (Grindem et al., 2015).

Not your basic team testing and training. No shortcuts or cookie cutter programs here. Team testing and training in our performance stream starts with a detailed assessment. Clinical testing looks at range of motions of key joints and pain provocations tests. This helps us understand the hardware and assess for injury risk factors. Strength testing gives us a glimpse into movement and strength symmetry to catch compensations that might be happening under the hood. Field testing (not shown here) is a battery of on-field sprint and agility tests to gauge how the clinical piece translates into performance. From there we pool athletes and create individual sport specific programs to reduce injury risk and improve performance for better, sustainable, on field,on ice, or on court performance. 🙌💪
The right way of doing things is rarely the easy way.
