FreshPeel or Deep Resurfacing — what’s the difference?

Both are CO₂ laser resurfacing treatments, but they’re designed for different skin concerns, treatment depths and levels of correction.

FreshPeel is our lighter, more superficial resurfacing option. It’s a great choice for clients looking to refresh their skin, improve mild pigmentation and sun damage, smooth texture and fine lines, and stimulate collagen — with less downtime.

Deep Resurfacing is our more intensive corrective treatment. It’s designed for clients with more significant sun damage, pigmentation, deeper lines, wrinkles and textural concerns who are looking for a more noticeable overall transformation.

So which one do you need?

It isn’t always about choosing the “stronger” treatment. Your skin type, concerns, treatment history, desired results and the amount of downtime you’re comfortable with all matter. We assess your skin and recommend the treatment depth that is most appropriate for your skin and your goals.

And sometimes, the two can be part of the same long-term skin plan — using deeper resurfacing for more significant correction and FreshPeel later to help refresh and maintain your results.

SUN SPOTS vs. MELASMA
They may look similar, but clinically they are very different — and treating the wrong type of pigmentation with the wrong technology can potentially make pigmentation worse.

WHAT ARE SUN SPOTS?
Sun spots, medically called solar lentigines, are benign areas of increased pigmentation strongly associated with cumulative sun exposure and photoaging. They commonly appear as individual, well-defined tan-to-brown spots on areas that receive significant UV exposure, including the face, hands, chest and shoulders.

Unlike melasma, solar lentigines are generally localized pigment lesions, which is why pigment-targeting laser treatments can be appropriate once the lesion has been properly assessed. Clinical studies support several laser and light technologies for solar lentigines, including Q-switched and picosecond pigment lasers. A 2025 systematic review of 41 clinical trials involving more than 3,200 patients found meaningful improvement with multiple laser and light modalities, although effectiveness and risk of post-inflammatory pigmentation varied by technology.

At Luxurious Laser & Cosmetics, treatment recommendations depend on the type of pigmentation, skin type, depth of pigment and the overall condition of the skin. PicoSure/pigment-targeting laser treatment may be considered for appropriate sun spots, while CO₂ resurfacing may be considered when pigmentation occurs as part of broader photodamage, uneven texture and overall resurfacing goals. CO₂ isn’t necessarily the first choice simply because someone has a brown spot; pigment-specific lasers have stronger evidence for targeting individual solar lentigines.

WHAT IS MELASMA?
Melasma behaves very differently. Rather than isolated spots, it commonly produces patchy, often symmetrical brown or grey-brown pigmentation, particularly on the cheeks, forehead, upper lip and other sun-exposed areas.

Melasma is a chronic, relapsing pigmentary disorder influenced by multiple factors. UV radiation is important, but visible light and hormonal influences can also contribute. This is one reason melasma can improve and then return even after apparently successful treatment.

And this is where choosing the correct treatment becomes especially important.

More aggressive does NOT automatically mean better for melasma.

Lasers are generally not considered first-line therapy for melasma. International expert recommendations reserve laser treatment primarily for carefully selected, refractory cases because inappropriate laser treatment can contribute to irritation, recurrence or post-inflammatory hyperpigmentation.

For that reason, we would not automatically recommend a Carbon Laser Peel or CO₂ resurfacing simply because pigmentation appears to be melasma. Proper assessment comes first.

Evidence-based melasma management typically begins with strict photoprotection and pigment-suppressing topical therapy, with prescription options managed by an appropriate medical provider when indicated. Current international expert consensus continues to identify supervised hydroquinone-based triple-combination therapy as a highly established treatment option, with other topical and procedural options considered according to the individual patient.

SUNSCREEN MATTERS — BUT THERE’S AN IMPORTANT DETAIL.

For melasma, UV protection alone may not always be enough. Research has demonstrated that visible light can contribute to pigmentation, particularly in darker skin types. Studies have found improved melasma outcomes when broad-spectrum sunscreen protection is supplemented with visible-light protection, such as iron-oxide-containing tinted sunscreen.

The takeaway?

Two brown patches can look very similar in the mirror while behaving completely differently biologically.

Sun spot ≠ melasma.
And laser ≠ the answer to every type of pigmentation.

CO₂ laser is an ablative resurfacing technology with established use in dermatology and aesthetic medicine. It can address concerns including fine lines and wrinkles, photoaging/sun damage, uneven texture, acne scarring and certain benign skin lesions.

By creating controlled microscopic zones of ablation and thermal injury, CO₂ laser removes damaged tissue while stimulating the skin’s natural healing response, including new collagen formation and collagen remodeling.

Treatment depth, settings and technique are selected according to the condition being treated and the individual patient.

WHAT IS CO₂ LASER RESURFACING — AND HOW DOES IT WORK?

Not all skin-rejuvenation technologies work the same way — and understanding how a treatment works is important when choosing the right treatment for your skin.

CO₂ laser is an ablative resurfacing technology. It uses a 10,600 nm wavelength that is highly absorbed by water in the skin, allowing controlled removal of microscopic areas of tissue while creating a thermal response within the skin.

With fractional CO₂ resurfacing, only a fraction of the treatment area is targeted at one time, leaving surrounding tissue intact to support the healing process.

As the skin repairs itself, a remodeling response occurs, including the production and reorganization of collagen. The goal isn’t simply exfoliation — it’s controlled resurfacing followed by regeneration and remodeling from within.

This is why CO₂ may be selected to improve concerns including:

• Fine lines & wrinkles
• Acne and other textural scarring
• Uneven skin texture
• Photoaging/sun damage
• Enlarged-looking pores
• Skin laxity and overall skin quality

Clinical research has demonstrated improvement in atrophic acne scars following fractional CO₂ resurfacing, with improvements continuing to be observed months after treatment. Histologic research has also demonstrated new collagen formation following fractional CO₂ treatment.

So how is CO₂ different from other technologies?

Microneedling creates controlled mechanical micro-injuries to stimulate the skin’s wound-healing response.

RF microneedling takes this a step further by delivering radiofrequency energy through needles into the dermis, creating controlled thermal injury while limiting disruption to the skin’s surface.

Non-ablative technologies primarily deliver energy beneath or through the surface without intentionally ablating the epidermis.

Ablative fractional CO₂ is different because it actually resurfaces microscopic portions of the skin while simultaneously producing a thermal remodeling response.

And there is clinical evidence behind the distinction.

A 2026 meta-analysis of 8 randomized controlled trials involving 249 patients compared fractional CO₂ with microneedling RF for post-acne scarring. Overall, fractional CO₂ produced greater scar improvement and higher patient satisfaction in the pooled analysis.

Treatment selection should take into consideration your skin type, concern, desired level of correction, medical history and tolerance for downtime.
