If someone wants to inject PCL under your eyes, walk away.

PCL, or polycaprolactone, is a biostimulatory product similar to Sculptra. It works by triggering your body to build collagen around it, which sounds great until you remember where it is being placed. The published recommendations for PCL explicitly state it should not go anywhere in the periorbital region.

Here is why that matters. The skin under your eyes is the thinnest on your entire body. Any lump, nodule, or granuloma will be visible and there is no dissolver to fix it. Unlike hyaluronic acid filler, this is not reversible.

PCL is not a bad product. It is being used in the wrong place, and some providers know that and do it anyway.

Human donor fat injected into your tear troughs. No reversal. No long-term data. Would you do it?

Lipoderma is cryopreserved human adipose tissue that gets injected to restore lost volume. Unlike HA, you cannot dissolve it. Unlike a traditional fat transfer, your body breaks down the donor cells and replaces them with your own fat over time. It sounds fascinating, and it genuinely is. But fascinating is not the same as proven.

The tear trough is one of the least forgiving areas on the face. Before I put anything irreversible there, I need five to ten years of real-world outcomes. We do not have that yet.

Ultrasound in aesthetics is being oversold.

I own an ultrasound. I have used it in my practice for four years. It has genuine value for identifying arteries in high-risk areas or investigating a suspicious nodule. But what I will not do is use it to validate a diagnosis that was never possible in the first place.

A patient with no history of jawline filler walked into another clinic convinced she had filler migration into her jowl. The provider confirmed it. That is not advanced medicine. That is confirmation bias dressed up as technology.

The tool is only as good as the expertise behind it. Anatomy knowledge, complication recognition, and clinical judgment will always matter more than the device in your hand.
