We are very happy to be reopening! Be assured that patient safety is our number one concern. We will be taking the following steps to ensure the safety of patients and staff:

Patients coming in will need to respond to a COVID-19 questionnaire and sign a COVID-19 consent form.

Please bring a mask and wear it when coming into the clinic.

Patients will be asked to sanitize their hands upon entry.

Patients only will be able to enter clinic. We ask that you do not bring anyone else with you.

Entrance is by appointment only.

Please contact Jan when you arrive and she will let you know when to enter - the front door will be locked.

We will be following the strictest guidelines to keep our environment as safe as possible. All staff will also be wearing masks/facial shields to ensure patient safety and all surfaces will be sanitized prior to each appointment.
