I've had a few post menopausal patients this week describe what we call in the bladder biz, "Key in the Door Syndrome." Yes, that's what we call it when you return home with your groceries and try to put the key in the door and get urgency. You shiver and shake (ok that's not part of the syndrome but you know what I mean). Women laugh (which will also make them leak) when I tell them what they have.
This condition is frequently referred to as urinary urgency/frequency symptoms, alternatively known as urge incontinence. It manifests when the sensation to urinate arises, and as you reach home and insert the key into the lock, a sudden intense urge results in leakage or a strong sensation of an imminent leak.
And it's common at menopause and beyond.
But what can you do about Key in the Door?
1. Stop what you are doing and remain calm.
2. Do 10-15 quick Kegels- it will calm your bladder down.
3. Do 'The Knack' a robust and precisely timed contraction of the pelvic floor muscles. It entails the pelvic floor muscles contracting just before and during a rise in downward pressure on the pelvic floor.
4. Once you bring in the bundles, make an appointment to see your doctor and ask them for a prescription for low dose localized estrogen therapy. It will help with the urge and the leak. And if you have va**nal dryness/painful s*x and/or recurrent urinary tract infections, low dose localized estrogen in the form of a cream, ring or tablet will help with that too. Bonus!

Dear Ladies!!
If you are peri-menopausal (the years leading up to menopause) or post menopausal (after you been without a period for one year) and you have any of the following:
Urinary Incontinence
Urgency
Frequency
Nocturia
Burning with urination
Recurrent Urinary Tract Infections
Vaginal Dryness
Ge***al itching
Vaginal Discharge
Painful s*x
Bleeding after s*x
Decreased va**nal lubrication during s*xual activity
Speak to your doctor about them prescribing LOW DOSE LOCALIZED ESTROGEN THERAPY in the form of a cream, ring or tablet.
These are symptoms of Genitourinary Syndrome of Menopause or GSM formerly known as Vaginal Atrophy but that doesn't quite do it justice so they name was changed. And localized estrogen is the only actual TREATMENT (as it replace the estrogen in the urogenital tract and the squamous epithelium of urethra, va**na, bladder etc)
Vaginal estrogen therapy is safe, effective and is LIFELONG therapy. It is inserted va**nally at night x 7 days, then twice a week at night on a Monday and Thursday or Sunday/Wednesday. It starts to work within a week or two and optimizes within 2 months.

Liza had low s*xual desire. Married 18 years, she loved her husband, found him attractive but lacked s*xual thoughts & fantasies. Ge***al sensation diminished, which accounted for by another medical condition. She rarely initiated s*x.
Liza's husband was upset but more importantly Liza was bothered.
Sounds like garden variety female low s*xual desire, a condition that affects approximately 44% of women.
But the circumstances of Liza's story made me question Female Sexual Interest & Arousal Disorder (FSIAD) formerly known as Hypoactive Sexual Desire Disorder or HSDD.
Sexual desire is cultivated through mindfulness & more so I made some suggestions for this busy entrepreneur to rule out FSIAD.
1. Mindfulness daily
2. Silver Balls - Run small silver balls under warm or cool water & then s*xt her husband. A simple strategy to kickstart arousal.
3. self love with the womanizer, a cl****al suckling device to release endorphins.
4. Wear gorgeous bras & underwear daily.
5. Wine, candles, massage oil
If s*xual desire isn't re-ignited, it may be something more like the medical condition Female Sexual Interest & Arousal Disorder.
