In FUE hair transplantation, donor management is not simply a question of how many grafts can be removed. The critical issue is how those extractions are distributed across the safe donor zone.
Each follicular unit removed creates a small circular wound and permanently reduces follicular density at that extraction site. If too many grafts are taken from a concentrated area, the remaining density can become visibly reduced, creating patchiness or an over-harvested appearance.
For that reason, extraction points should be distributed as evenly and sparsely as possible, while respecting the boundaries of the permanent donor zone and the individual patient’s baseline density, hair calibre, follicular-unit composition and scalp characteristics.
The principle is straightforward:
The greater the concentration of extraction in one area, the greater the local reduction in donor density.
By spreading extractions over a broader safe area, the reduction in density is distributed more uniformly. This helps preserve the visual homogeneity of the donor region and may leave more usable donor reserve for future procedures.
Punch diameter also matters. Smaller punches can reduce the surface area of each extraction wound, but punch size must still be appropriate for the patient’s follicular-unit anatomy to minimise transection and unnecessary tissue trauma.
