Real patient case: when tracking is lost, what do you do next?

At aligner 15, the patient was rescanned and the current tooth positions were compared with the original plan.

The result showed a clear discrepancy:
→ Anterior intrusion was achieved
→ Planned posterior extrusion was not

That difference matters.

Instead of simply moving on to the next set of aligners, the doctor can reassess treatment from the patient’s current position and decide what the next step should be.

What’s actually happening under an attachment?

A canine is staged for a clean rotation.
The aligner tracks perfectly.

And on the next scan, that same tooth has barely turned — instead, it’s a fraction of a millimeter shorter.

This is what happens on any rounded surface under compression without a flat stop to redirect the force: it squeezes out from under the load, the aligner lifts, and the force resolves apically — the same way a watermelon seed shoots out from between two fingers instead of just rotating in place. What was planned as rotation shows up on the next scan as intrusion.

Canines and premolars are rounded by nature. So every clean rotation on those teeth depends on giving the aligner a flat, precisely angled surface to push against.

That’s the entire job of an attachment — a specific geometry engineered to convert a squeeze into a rotation.
