Surgery = discectomy (remove disc piece pressing on nerve).
· Only if: 6+ weeks of severe pain/weakness that therapy didn't fix, or emergency (loss of bowel/bladder control – go to ER now).
· Main types: Microdiscectomy (gold standard, 1–2" cut) or Endoscopic (tiny incision, faster recovery).
· Upside: Quick pain relief.
· Downside: ~6% risk of reherniation.
· Recovery: Walking same day; desk job ~1–2 weeks, heavy work ~4–8 weeks.

summary of hand signs in aortic regurgitation

✋ Hand Signs – Bedside Clues

1. Water‑hammer pulse (Corrigan’s sign)

· How: Raise patient’s arm, feel the radial pulse.
· What: Bounding pulse with a rapid rise and sudden collapse — like water rushing through a hose.
· Why: Wide pulse pressure from rapid aortic runoff.

2. Quincke’s sign

· How: Gently press on the fingernail tip.
· What: Alternating redness and blanching of the nail bed with each heartbeat.
· Why: Capillary pulsation due to hyperdynamic circulation.

💊 Bottom Line (No Tables, No Tablets)

· No medication fixes the valve.
· Definitive treatment = surgery (valve replacement).
· Drugs (vasodilators, diuretics) are only temporary to control symptoms before surgery.
· Surgery is needed if: symptoms (dyspnea, palpitations, angina) appear, or ejection fraction drops below 50%, or LVESD > 50 mm.

Ever notice red, raised lines appearing on your skin after scratching, rubbing, or wearing tight clothes? That's not a food allergy or a mysterious rash – it's a common, harmless condition called dermatographia, which literally means "skin writing."

What causes it?
Your immune system releases too much histamine in response to friction or pressure on the skin. This causes fluid to leak from tiny blood vessels, resulting in itchy, swollen welts that trace exactly where you scratched.

Common triggers: tight clothing, belts, jewelry, towels, bedsheets, rough fabrics, stress, heat, or exercise.

Timeline: symptoms appear within 5–10 minutes of friction and usually fade on their own within 30 minutes to 2 hours.

What to do about it:

For mild or occasional symptoms, no treatment is needed. Just avoid scratching, wear loose cotton clothing, and use a gentle moisturizer to keep your skin calm.

If itching is moderate, over-the-counter non-drowsy antihistamines like loratadine, cetirizine, or fexofenadine can help. Take them daily during flare-ups.

For severe or persistent cases, see a dermatologist. They may prescribe stronger antihistamines, hydroxyzine, or other medications like montelukast.

For immediate itch relief, apply cool compresses, calamine lotion, or 1% hydrocortisone cream for short-term use

⚠️ Emergency warning:
If you experience swelling of the face, lips, or tongue, difficulty breathing, or wheezing – these are NOT typical of dermatographia and require emergency care immediately.
