New Approach to Knee Replacements Prioritizes Natural Leg Alignment

Although medical science has continuously evolved, many people remain less than fully satisfied following knee replacement surgery. One potential reason for this may lie in the surgical method traditionally employed—a technique that fundamentally involved straightening the leg axis. However, a shift in thinking is now underway.

For many decades, the standard practice in knee replacement surgery was to completely correct deformities such as bow-legs or knock-knees. The objective was to achieve a leg that was as straight as possible, featuring a vertical axis extending from the hip to the foot. However, this approach gave little consideration to the fact that most people do not naturally possess a perfectly straight leg axis.

Historically, the primary rationale behind this approach stemmed from the materials used at the time. To enhance the durability of the prosthesis and minimize wear and tear, the load was required to be distributed as evenly as possible. Today, however, modern materials and improved implant designs are paving the way for new approaches. Even if the upper and lower components of the prosthesis do not align in a perfectly straight line, the resulting abrasion is significantly reduced.

Consequently, there is a growing trend toward preserving the patient's original, natural leg alignment. Rather than completely straightening the leg, surgeons now focus on correcting only those structural changes that have resulted from joint degeneration. The goal is to create an artificial knee that feels as natural as possible and goes virtually unnoticed during everyday activities.

This evolution is further supported by robot-assisted surgical techniques. These technologies enable highly precise customization of the prosthesis while also taking into account the tension of the ligaments and surrounding soft tissues. As a result, the joint can be implanted with greater precision and tailored more specifically to the individual patient.

Of particular importance in this context is the knee's so-called ligamentous apparatus. If a pronounced bow-leg or knock-knee deformity is overcorrected during surgery, the ligaments and tendons often require extensive modification. This can subsequently lead to pain, instability, or an unnatural sensation during movement. The fewer interventions required on the soft tissues, the faster many patients recover after surgery.

However, studies also show that not everyone automatically experiences an improved quality of life following a knee replacement. Experts therefore increasingly believe that individually tailored care is crucial in determining whether the artificial joint subsequently feels as natural as possible.

German knee experts and their team are conducting research on knock-knees (X-legs) and bow-legs (O-legs).
Regarding knock-knees, our understanding is not yet sufficiently precise.
Regarding bow-legs, studies have shown:
Modern knee prostheses can withstand a slight bow-leg alignment of five degrees.

Whether more pronounced bow-legs are also feasible—without causing the prosthesis to fail prematurely—is still under investigation.

At the Klinikum rechts der Isar in Munich, doctors are utilizing a smartphone app.
The app records the movements of patients suffering from knee osteoarthritis or those who have received artificial knees.

In this way, the doctors aim to determine:

* Which patients benefit from which specific type of artificial knee
* Which alignment yields the best results
* What issues or signs of progress arise

One patient interviewed for this report underwent surgery at the Pius-Hospital in Oldenburg. He is extremely satisfied with his new knee.
Should he require another knee replacement in the future, he would once again opt for a slight bow-leg alignment.

For a long time, obstructive sleep apnea was understood primarily as a mechanical disorder: during sleep, the upper airways narrow or close off, causing breathing to repeatedly pause and blood oxygen levels to drop. For this reason, nocturnal positive airway pressure therapy using a CPAP (Continuous Positive Airway Pressure) mask remains the standard treatment to this day. However, numerous studies now indicate that the condition is underpinned by significantly more complex neurophysiological processes.
Against this backdrop, medications originally developed for other medical indications are coming into focus. Carbonic anhydrase inhibitors, in particular, are currently being investigated intensively. These agents could stabilize the autonomic respiratory control system, thereby reducing the frequency of nocturnal breathing pauses. Furthermore, recent research findings suggest that they may not only influence nocturnal respiratory control but also potentially directly improve the structural stability of the upper airways. This could open up new therapeutic avenues—particularly for patients who cannot tolerate CPAP therapy or who do not use it consistently in their daily lives.
Sleep Apnea: More Than Just Airway Narrowing
While anatomical factors—such as excess weight, fat deposits in the pharyngeal region, or a recessed lower jaw—contribute significantly to the development of sleep apnea, these factors alone cannot fully explain the clinical picture of the disease.
Neuromuscular mechanisms also play a crucial role. In many affected individuals, for instance, the activity of the genioglossus muscle—the tongue muscle responsible for keeping the upper airways open—is diminished. Additionally, numerous patients exhibit a lowered arousal threshold, making them more susceptible to airway narrowing. Moreover, a disturbance in the stability of respiratory regulation is frequently observed.
Of particular significance in this context is an elevated "loop gain." This concept, derived from control theory, describes the sensitivity of the body's intrinsic respiratory control system. In the case of an overactive feedback loop, the organism reacts excessively to even minor changes in oxygen or carbon dioxide concentrations. This can lead to unstable breathing, which oscillates between phases of hyperventilation and pauses in breathing. This mechanism plays a particularly significant role in certain forms of sleep apnea. The problem is further exacerbated during REM sleep, as muscle tone decreases significantly during this phase, thereby facilitating more pronounced drops in nocturnal oxygen levels.
