The patient is a lovely 12 girl who slipped by the swimming pool and fractured her tooth 6 months ago with a history of a failed composite restoration.

Restoring a severely mutilated maxillary molar ( #26) using a modern biomimetic overlay concept—where preservation, adhesion, and biomechanics drive every decision 🦷
Instead of traditional full-coverage reduction, the treatment followed contemporary overlay preparation principles aimed at reinforcing the remaining tooth structure while controlling stress distribution.
🔬 Clinical & design highlights:
▪️ Selective caries removal endpoints
Infected dentin was removed while affected dentin was preserved when appropriate, maintaining pulpal vitality and maximizing substrate quality for bonding.
▪️ Immediate Dentin Sealing (IDS)
Freshly cut dentin was sealed immediately after preparation to improve bond strength, reduce post-operative sensitivity, and protect the dentin–pulp complex throughout the indirect workflow.
▪️ Modern overlay preparation design
– No traditional ferrule or circumferential axial reduction
– Rounded internal line angles to reduce stress concentration
– Cuspal coverage only where structurally indicated
– Butt-joint enamel margins for predictable enamel bonding
– Supragingival margins whenever possible for better isolation and long-term maintenance
– Occlusal reduction guided by functional demand, not arbitrary numbers
▪️ Dentin replacement phase
Lost dentin was biomimetically replaced using adhesive restorative materials to re-establish internal stiffness and act as a stress-absorbing foundation.
▪️ Enamel replacement via bonded overlay
The overlay functions as an enamel substitute, restoring occlusal anatomy, cusp support, and marginal seal while maintaining maximum remaining tooth structure.
The end goal was not just to restore shape—but to restore biomechanics, control crack propagation, and extend the life of a compromised tooth using adhesive principles rather than mechanical retention.

Yet another case,
A fine gentleman that had flown all the way from KSA had the chance to get complete smile glowup within a broaders full mouth rehabilitaion case in collaboration with .sa3id
The case what full of hurdles and set backs, with a limited time of 10 days, lots of endo retretment , occlusal plane disharmony, denta class II in the anterior reagion and gingival recountouring challange.
Nevertheless, the case was succefully managed. All thnaks to gad, hard work and proper diagnosis, excution and delivery

🦷 Overlay / Endocrown Hybrid – Tooth #46 🔬✨
A structurally compromised, endodontically treated lower first molar ( #46) restored using an overlay–endocrown hybrid approach — combining cuspal coverage with pulp chamber retention for optimal biomechanics and adhesion.
This case required a restorative strategy that balances macro-retention from the pulp chamber with adhesive bonding to remaining enamel and dentin, allowing us to avoid full-coverage crown preparation while reinforcing weakened cusps.

🦷 Ceramic Overlay for Tooth #17 – A Biomechanical Approach 🔬
In this case, we tackled a large amalgam restoration on tooth #17, which had recurrent decay. The goal was to restore both form and function while maintaining maximal tooth structure preservation. 💪
Cavity Design Considerations:
Caries Removal 🦠: A complete debridement of the decayed amalgam and surrounding tissue was essential to eliminate residual bacterial contamination.
Internal Geometry 🔄: The cavity was carefully designed with rounded internal line angles to ensure stress distribution and prevent fracture initiation.
Conservative Tooth Reduction ✂️: Adequate reduction of the tooth structure was done while maintaining sufficient thickness for the ceramic overlay, ensuring optimal adhesive bonding.
Adhesive Bonding 🔗: After proper isolation, we used total-etch bonding for optimal adhesion between the ceramic material and tooth structure, ensuring longevity and functional stability.
