Nobody told you this at induction. Nobody mentioned it in the job description. It wasn't in the contract, wasn't covered in the training, wasn't part of the onboarding.
You were told about the uniform, the shift patterns, the health and safety procedures โ and not one word about what eight hours a day on hard floors would do to your feet over time.
So let me tell you now. The three professions I see most: ๐๐๐๐๐๐, ๐๐๐๐๐๐๐๐. ๐๐๐๐๐๐ ๐๐๐๐๐๐๐.
Different environments, different pressures, different reasons for being on their feet all day. The same presentations arriving at Pedicure Studio by Alexandra Martin with the same slightly surprised expression โ surprised, specifically, that feet which have been working this hard for this long have finally decided to make themselves known.

๐ Studies on chronic foot pain among nurses consistently report prevalence rates between 23% and 52% โ and that 24% of nurses have been absent from work in the past 12 months due to foot and ankle pain, making it the second most common reason for absenteeism among healthcare professionals.
A profession built on caring for others, systematically not caring for the feet that make it possible.

๐๐๐๐๐ก๐๐ซ๐ฌ stand for the majority of their working day on classroom floors that are typically concrete or composite โ surfaces that absorb no impact and return every kilogram of force directly upward through the foot.
๐ A study of female workers in standing jobs found that 76.3% experienced foot and ankle pain, with the strongest associations being prolonged standing hours and inappropriate footwear. The shoes most teachers wear (smart enough for a professional environment, rarely chosen for clinical foot support) sit squarely in the "inappropriate" category.

๐๐๐ญ๐๐ข๐ฅ ๐ฐ๐จ๐ซ๐ค๐๐ซ๐ฌ spend four to ten hours of every shift on their feet, with over a third spending eight to ten hours upright.
๐ A UK study found that 45% of hospitality and retail workers experienced physical pain before the pandemic - and that after returning to work following the furlough period, 52% reported foot pain, an increase of 11%. The telling detail: during lockdown, when they weren't standing, the pain dropped significantly. When they came back, it returned โ and exceeded previous levels.
The feet know what the job costs. The question is whether anyone is paying attention.

The foot is โ๏ธ not designed for static load. It is designed for movement โ the dynamic, shifting, redistributing motion of walking, where pressure moves across the heel, the arch, the ball, and the toe in sequence, and no single structure bears the full load for long.
Standing changes this entirely.
In prolonged static standing, pressure concentrates ๐ in fixed points โ the heel, the ball of the foot, the inner arch. The plantar fascia, the band of connective tissue running along the bottom of the foot, is placed under sustained tension rather than the rhythmic loading it's built for.

Pain significantly affected daily functioning in nurses studied: 39.8% avoided walking long distances, 39.3% changed their walking style, and 42.8% avoided prolonged standing outside of work.

Most people in these professions come to me when they can no longer manage:
๐ when the plantar fasciitis has been present for six months;
๐ when the ingrown nail has become infected;
๐ when the calluses are severe enough to alter their gait;
๐ when the problem has moved from background discomfort to foreground pain that is affecting their ability to do their job.

Preventive clinical foot care for people in standing professions is not a luxury. The same logic that says
โช a nurse should have occupational health support,
โช a teacher should have ergonomic assessment,
โช a retail worker should have appropriate footwear guidance..that logic extends to the feet those professionals stand on for the majority of their working lives.
A session every two to three months: assessment, maintenance, early intervention before anything becomes established.
The kind of care that means the problem never gets to the point where it affects your shift, your absence record, or your ability to do the job you trained for.

The pool was in July. The nail changed in August. She came to me in October 2025 - it didn't have to take that long.
I see this pattern every single year without exception: British summer arrives. People
โช swim โ in leisure centres, in holiday hotels, in the outdoor lido that opens for eight weeks and feels like a different country. They
โช walk barefoot on pool surrounds, in communal changing rooms, through the warm wet corridors between the showers and the exit.
Then September comes. And so do they - with ๐ nails that have changed colour. With ๐ skin between their toes that won't settle. With the ๐ specific, slightly embarrassed expression of someone who knows something has been happening for a while and has been hoping it would resolve on its own.
It hasn't. And the longer they waited, the longer the treatment now takes.

The assumption most people carry is that chlorinated pool water is clean water โ protected, safe, hostile to infection. It is, to a point.
Scientific studies indicate that fungal spores can survive in treated pool water for up to 48 hours, though their viability decreases over time with proper chlorination levels.
The pool surround (the wet concrete you walk on between the changing rooms and the water) is a different matter entirely. Warm, consistently damp, walked on by hundreds of bare feet per day: it is precisely the environment in which dermatophytes thrive.
Dermatophytes are responsible for over ๐๐% ๐จ๐ ๐๐ฎ๐ง๐ ๐๐ฅ ๐ญ๐จ๐๐ง๐๐ข๐ฅ ๐ข๐ง๐๐๐๐ญ๐ข๐จ๐ง๐ฌ in the UK. They pe*****te the nail plate or nail bed through small cracks or trauma โ a stubbed toe, a nail cut slightly too short, a small area of skin breakdown between the toes that you didn't notice and wouldn't have thought significant. The entry point doesn't need to be dramatic. It needs only to exist. And in summer, it almost always does.

This is the part that explains October :)
Onychomycosis (๐๐๐๐๐๐ ๐๐๐๐ ๐๐๐๐๐๐๐๐๐) does not announce itself immediately.
The infection begins ๐ at the edge of the nail or underneath it, and spreads inward slowly, feeding on keratin, the protein that gives nails their structure.
In the early weeks, there may be no visible change at all. The nail looks normal. Nothing hurts. Nothing feels different. By the time the nail begins to discolour (yellowing, thickening, a change in surface texture) the infection has typically been present for weeks, sometimes longer.
By the time it becomes visually obvious enough for someone to think "I should probably get that looked at," it is already established across a meaningful portion of the nail plate.
Treatment may take at least 12 months, and some patients may require prophylactic treatment to prevent recurrence.
Twelve months. For a condition that, caught early, responds significantly faster to clinical treatment.
The pool was in July. The nail changed in August โ which means the infection was already progressing. She came to me in October, by which point the infection had been developing for approximately three months without intervention - 3 months in which the treatment window was narrowing.

September is consistently the busiest month at Pedicure Studio by Alexandra Martin. Summer does NOT cause foot problems โ it reveals and accelerates them.
The warmth,
the communal facilities,
the barefoot walking,
the holiday footwear..all of it creates conditions that allow problems to establish and spread that winter's closed shoes would have contained.
What I see in September, clinically:
โก ๐ญ๐๐๐๐๐ ๐๐๐๐ ๐๐๐๐๐๐๐๐๐๐ at varying stages of establishment โ some caught early enough to respond quickly, some requiring considerably longer management because they've been present since June and nobody recognised the early signs.

๐๐ โ athlete's foot โ that began between the toes at a leisure centre in July and has since spread to the nail edge, which is the most common route of nail infection.

๐๐๐๐ ๐๐๐๐-๐๐๐-๐๐๐๐๐๐๐ ๐๐๐๐๐๐๐ since August โ sometimes the right product, almost always applied to a nail that hasn't been properly prepared to receive it, which means the active ingredient cannot pe*****te to where the infection actually lives.
All of it treatable. All of it faster to treat when it's caught early.

Early is not when it becomes obvious. Early is when you first notice the nail looks slightly different:

Most people find this out the hard way - in the way that gradual things always announce themselves, through the accumulation of small inconveniences that one day stop being๐ป๐๐ ๐ ๐๐๐๐ ๐๐๐๐ข๐โ to ignore.

Your feet in your 40s are capable, competent, and beginning to tell you things you haven't heard before.
โก๏ธ The fat pads (the natural cushioning beneath your heel and the ball of your foot) begin their gradual decline in this decade. These pads are your foot's ๐ โ๐๐๐ ๐๐๐ ๐๐๐๐ก๐๐๐ ๐ ๐ฆ๐ ๐ก๐๐.
As they thin, the bones beneath them move incrementally closer to the ground. The result is not dramatic pain, but a new kind of tiredness in the feet at the end of the day - a soreness that wasn't there at 35 and that rest doesn't quite fully resolve.
โก๏ธ Skin hydration decreases. The heel skin that moisturiser managed easily at 30 now requires more consistent attention to stay supple. Collagen loss leads to thinner skin and reduced flexibility, increasing the risk of fissures.
โก๏ธ Nails begin to grow more slowly and, in some cases, to thicken subtly โ a change so gradual it's easy to attribute to anything other than what it is.

The 50s are when previously manageable foot issues can turn into long-term conditions that interfere with daily life. This is because the changes of the 40s continued without clinical management.
โก๏ธ Fat pad atrophy (the gradual thinning of protective cushioning under the heel and forefoot) progresses. Research published in 2024 confirmed that age-related changes in heel fat pad structure alter its mechanical response, diminishing shock absorption and contributing to heel pressure and pain.
When the fat pads diminish, additional stress falls on the skin, which dries and cracks. This is also the reason feet feel sore at the end of the day โ the bones are simply closer to the ground than they were.
โก๏ธ For women in this decade, hormonal changes following menopause can accelerate fat pad thinning. Declining oestrogen levels specifically affect the body's ability to maintain fatty tissue beneath the skin โ a connection that is rarely made until someone explains it directly.
โก๏ธ Degenerative joint conditions such as arthritis in the toes, ankles, or midfoot may emerge.
โก๏ธ Calluses and corns become more frequent due to altered gait and pressure distribution.
โก๏ธ Nail thickening becomes more pronounced.

None of what I've described is inevitable in the sense that it cannot be managed.
What is inevitable, without clinical attention, is that the changes of the 40s become the problems of the 50s, and the problems of the 50s become the conditions that limit life in the 60s.
The people who come to me in their 60s with the most straightforward clinical picture are, almost without exception, the people who started paying attention in their 40s - because they understood that feet are not a part of the body that maintains itself - and that the cost of prevention is always lower than the cost of treatment.

There's a specific kind of resignation I recognise immediately when someone sits down in my chair.
They take their shoe off slowly. Sometimes with a brief apology. They decided, somewhere along the way, that the nail was simply how things were going to be now.

โช yellow-brown thickening of the nail plate with
โช elongation and increased curvature..it is more commonly known as ram's horn nail โ a description that, once you've heard it, is difficult to forget.
It develops from several ๐๐จ๐ง๐ฏ๐๐ซ๐ ๐ข๐ง๐ ๐๐๐ฎ๐ฌ๐๐ฌ:
๐ previous injury to the nail or nail matrix that triggers abnormal growth;
๐ chronic fungal infection that weakens nail structure over time;
๐ poor peripheral circulation;
๐ ill-fitting footwear that exerts sustained pressure on the nail plate; and, in many cases, simply
๐ the accumulation of years without specialist clinical care.
It is not vanity, nor neglect in the simple sense. It is a nail that has been responding, over a long period, to conditions that nobody addressed at the clinical level โ because nobody explained that there was a clinical level to address.

The belief that a thickened, curved nail is a permanent condition comes from a reasonable place:
โช You've tried cutting it differently.
โช You've tried filing it.
โช You've perhaps tried pharmacy treatments that made no visible difference.
The nail continued doing what it was doing โ growing thicker, curving further, pressing against the shoe in ways that ranged from uncomfortable to genuinely painful. You ran out of things to try. You concluded there was nothing left.
What you didn't try was clinical debridement by someone trained specifically to work with nail pathology.
Onychogryphosis is recognisable by pronounced abnormal thickening of the nail, which takes on a yellowish-brown hue and curves downward as it grows โ a condition that can develop from untreated nail plate hypertrophy and nail bed hypertrophy over time.

๐ I am identifying whether the thickening has a fungal component requiring additional treatment, whether the curvature is creating pressure points that are affecting adjacent toes, whether there are signs of underlying conditions that need monitoring or referral.
๐ And I am beginning โ sometimes gradually, over several sessions โ the process of redirecting growth.
The nail matrix, which produces the nail plate, can be influenced by the way the nail is managed clinically over time. A nail that has been curving in the wrong direction for years does not straighten in one appointment. But with consistent, correct clinical management,
