Sciatica is pain caused by an irritated nerve. It usually affects one side, starting from the lower back or bottom and spreading down one leg.
Symptoms of sciatica include shooting pain that starts in your lower back or bottom and runs down the back of your leg, often to your foot and toes.
Common treatments for sciatica include exercises and stretches. Other treatments like injections or surgery may help if it does not get better.
You get sciatica when something presses or rubs on the sciatic nerve. The most common cause is a slipped disc in your spine.
Here the study from 2009
University Science Malaysia, a prospective observational study was carried out on 126 consecutive patients who attended the Pain Management Unit at Hospital University Science Malaysia, Kelantan, between 1st January and 31st December 2009 for chronic back pain (i.e. lasting more than 6 months).
Myofascial pain syndrome (MPS) is a trigger point-induced regional musculoskeletal pain disorder affecting one or more muscles or groups of muscles. A myofascial trigger point is defined as a hyperirritable spot, usually within a taut band of skeletal muscle or in the muscle fascia, which is painful on compression and can give rise to characteristic referred pain, motor dysfunction, and autonomic phenomena.
MPS is a common cause of pain and dysfunction in the musculoskeletal system. It accounts for 20% to 95% of patients with musculoskeletal pain presenting at general medical clinics and pain management centres. Despite the prevalence of MPS, awareness among clinicians is still inadequate, which results in this condition often being misdiagnosed or underdiagnosed and thus under-treated
MPS prevalence among chronic back pain patients was significantly high, with female gender being a significant risk factor. With proper diagnosis and expert management, MPS has a favourable outcome.
Myofascial Cupping’ or ‘Cupping’ an ancient Chinese therapy in which a glass, plastic or bamboo cup is applied to the skin then the pressure in the cup is reduced (either by heat or suction) in order to draw and hold skin and superficial muscle tissue/fascia inside the cup. Sometimes, while the suction is active, the cup is moved, causing the skin and muscle to be pulled. This is called ‘gliding cupping’. Cupping is applied to certain acupuncture points as well as to parts of the body that have been affected by pain or muscle/fascia tightness where the pain is deeper than the tissues to be pulled. The cupping process draws blood from your vessels and into your tissue. Your body thinks it’s been injured, so the theory is that it ‘jumpstarts’ a postive inflammatory response, mobilizing antibodies to the area to try to heal it.
Cupping for athletes can aid in the recovery process as increased blood flow from cupping may help accelerate the repair of the damaged muscle fibres that result from hard training.

🔹 Piriformis muscle syndrome 🔹

Pain that prevents normal functioning?
These are pain ailments more commonly associated with sciatica. However, are you sure? Very often, these symptoms are caused by piriformis muscle syndrome.
What is it ❓
The piriformis muscle is a small structure, but due to its proximity, it is of great importance.
It is located deep under the gluteus maximus. It runs from the sacrum to the trochanter of the femur. It is the external rotator of the thigh, abducts and straightens the lower limb and stabilizes the hip joint.
What is the problem❓
The sciatic nerve runs just below the piriformis muscle. So the nerve that supplies the posterior part of the lower limb. The nerve emerges above the piriformis muscle
The tension of the piriformis muscle can irritate the sciatic nerve, which will cause symptoms identical to sciatica. However, it will not be true sciatica!
What's more. Just above the piriformis muscle comes the superior gluteal nerve, and again, irritation of the nerve will project pain, this time to the buttock and hip.
So we can see that the piriformis muscle can really mess up, despite its small size.
What does therapy look like?

We release the tension of the muscles in the area and breake down fascial scar tissue buildup around that area.

If you have ever seen an Olympic athletes with dark red circles on their back, you have seen "Cupping." But there is an even older Traditional Chinese Medicine (TCM) technique that looks far more brutal, yet is profoundly effective: Gua Sha (or "Scraping").
In Western physical therapy, it is now known as Instrument-Assisted Soft Tissue Mobilization (IASTM)
To the uninitiated, it looks like you are severely bruising the skin. But biologically, you are hacking the body's repair system.
Petechiae vs. Bruising
A normal bruise is caused by blunt force trauma that crushes blood vessels, causing deep internal bleeding and tissue damage.
Gua Sha is different. By repeatedly scraping a smooth stone over lubricated skin, you create intentional, highly controlled friction. This pulls stagnant, deoxygenated blood out of the deep muscle capillaries and up to the surface of the skin, creating tiny red or purple dots called Petechiae (in TCM, this is called the "Sha").
You aren't crushing the tissue; you are essentially vacuuming the stagnant metabolic waste to the surface.
The HO-1 Enzyme Miracle
Why purposely create petechiae? Because of the biochemical response.
When your immune system detects these tiny pools of blood at the surface, it panics. It thinks there is a massive injury.
To manage this "fake" injury, your cells massively upregulate an enzyme called Heme Oxygenase-1 (HO-1).
HO-1 is one of the most powerful antioxidant and cytoprotective enzymes in the human body. It breaks down the heme (from the red blood cells) into carbon monoxide and biliverdin, which act as extreme, localized anti-inflammatories.
The Hack: The HO-1 floods the entire area. It doesn't just clear the red marks; it penetrates deep into the muscle fascia, putting out the fire of chronic, decades-old inflammation and tension that a normal massage could never reach.

It runs from the upper cervical vertebrae (C1-C4) down to the top inner corner of the shoulder blade (scapula).
Function:
Elevates (lifts) the scapula
