The DAO — the muscle that pulls your frown down!

While the exaggerated expressions I’m making here don’t exactly look like my everyday pose 😂, they’re simply to demonstrate the difference between having the muscle treated and leaving it untreated. The extremes help show the impact this muscle can have on facial balance.

The DAO can contribute to a downward pull in the lower face, which may accentuate areas such as the corners of the mouth and contribute to the appearance of jowling over time. Including this area as part of a treatment plan can help create a more harmonious and balanced look while still respecting your natural facial movement.

No, I’m not calling my beautiful client a horse! 🐴😂

I was recently explaining temple hollowing and the ageing process to a client. She compared it to how her horses age, and surprisingly, the comparison was pretty accurate!

As we age, particularly from our 40s onwards, we can lose volume in the temple area—the space between the outer brow and the upper cheek beside the eye. This can create a hollowed appearance and contribute to a more tired or aged look.

Using advanced dermal filler techniques and careful assessment, we can restore volume to this area and create a softer, more youthful appearance. Because the temples are a delicate area with important blood vessels, treatment is performed with precision and may sometimes be carried out over two or three appointments, spaced apart, to achieve the safest and most natural-looking results.

Every treatment plan is tailored to the individual, ensuring beautiful, balanced results.

Treating Nasolabial Folds: A Three-Stage Approach

Every treatment begins with a full facial assessment. In this case, the client had more significant volume loss and tissue descent on the left side, so this was addressed individually to create better overall balance and facial harmony.

Stage 1: Structural Support
Dermal filler was placed deeply at the level of the piriform fossa to provide foundational support to the midface and surrounding tissues. Restoring support in this area can help soften the appearance of the nasolabial fold by improving tissue positioning and reducing shadowing.

Stage 2: Softening the Fold
A cannula was used to carefully treat the tissue behind the crease. The passage of the cannula can create a small degree of tissue disruption and release of tethering, which may contribute to tissue remodelling during healing. Filler was then placed beneath the fold to provide support and volume, helping to soften the crease. Hyaluronic acid filler also attracts water, which can contribute to a smoother and more hydrated appearance.

Stage 3: Refining Deeper Static Lines
Finally, small amounts of dermal filler were placed directly into the deeper static lines to further soften their appearance and improve overall skin smoothness.

By combining structural support, fold correction, and targeted line refinement, we were able to achieve a natural-looking improvement while maintaining the client’s unique features.

Maintaining Results
To maintain results and continue improving skin quality, I would recommend a consistent home skincare routine alongside professional microneedling treatments. Microneedling can help stimulate natural collagen production, improve skin texture, and support overall skin elasticity.

While dermal filler has achieved an excellent result here, it is a treatment that should be used thoughtfully and in moderation. The goal is always to restore balance and support, rather than create excess volume, ensuring natural-looking results that enhance rather than alter the client’s natural beauty.
