Do you have any cuts or abrasions in the eye area?

Do you suffer from Eczema, psoriasis or any other skin disorder or disease in the eye area?
Have you had any recent eye operations or scar tissue?
Do you suffer from eye diseases, disorders or infections including conjunctivitis,blepharitis,watery eyes,styes,bordeola,cyst,dry eye syndrome,cataract,glaucoma?

Have you previously had eyelashe extension treatment and experienced an adverse reaction?
Patch test is required 48/24 hours before treatment.
