⭕️it is used to blame individuals
⚠️a datix has been raised against you can you explain yourself in writing
⚠️i feel this should be escalated because it involves you
⭕️ When questioning becomes interrogative
⚠️why is it that he refuses treatnent if u are on shift, it must be you with poor communication skills
⚠️why many falls when its you on shift
⚠️why is he violent when you are in that bay
⭕️When disciplinary threats/language follows a report instead of supportive discussions.
⚠️this is becoming too much , you are not the only new nurse, i think HR should know
⚠️management need to know who did the mistake before we close this case
⭕️Lack of feedback, making it look like a surveillance tool or evidence rather than improvement
⚠️you have done so many mistakes, do you know your PIN is at risk, so many datix with your name but none of them was ever duscussed with you
⭕️If its used inconsistently or selectively making one feel psychologically unsafe in the workplace
⚠️when same incident happens and no datix is done
⚠️when senior staff are not datixed but only junior ones
⚠️when people of colour are datixed and some are not
