A lot of my patients have quietly stopped wearing dresses or shorts because of thread veins on the legs. It is rarely the veins themselves that bother them most. It is the years of not wanting to be looked at.

Microsclerotherapy is the treatment I most often suggest for thread veins on the legs. A very fine solution is injected directly into the affected veins, which causes them to close down and gradually fade in the weeks that follow. It is a long-established medical treatment, used in vascular medicine for decades, and is not the same as laser treatment for the same problem.

The treated veins typically settle and pale over the following weeks, although results vary and some veins respond more readily than others. A course of one to three sessions is common, depending on how much there is to treat. Bruising and tenderness for a few days afterwards are normal, and compression hosiery is worn for a short period to support the result.

Not everyone is a candidate. There are some medical reasons, including pregnancy and certain vascular conditions, why microsclerotherapy would not be suitable, which is one of the reasons it is doctor-assessed at the clinic before any treatment is offered.

A lot of patients come in around their late 40s or 50s saying the same thing in slightly different ways. The skin feels drier, looser, less resilient than it used to. Make-up sits differently. The neck and the jawline are quieter giveaways than the face itself.

What is happening is largely hormonal. As oestrogen levels fall through perimenopause and menopause, the skin loses collagen and elastin more quickly than the body replaces it, hydration drops, and the support structure beneath the surface gradually softens. None of this is in your imagination, and none of it is your fault.

NeoGEN is the treatment I most often suggest for skin at this stage. It is nitrogen plasma energy, which is different from a laser and different from a chemical peel. It works by encouraging the skin's own regeneration from within: collagen and elastin production over the weeks and months following each session. It is suitable for all skin types, including darker skin tones, which is not true of every energy-based device.

NeoGEN is delivered as a programme rather than a one-off session. Profhilo® is the treatment I most often suggest for patients who want their skin to feel and look healthier from the inside, rather than reshaped from the outside. It is not a dermal filler, and that distinction matters.

A filler adds volume to a specific area. Profhilo® works differently. It releases hyaluronic acid slowly into the skin, encouraging your own collagen and elastin production over the weeks that follow. The aim is improved skin quality: better hydration, a softer firmness, a quiet glow that comes from healthier skin rather than added product. May is when the diary tends to wake up. Weddings, long-overdue lunches, the slow run-up to summer.

Anti-wrinkle injections are the most asked-about treatment at the clinic, and they are also the most misunderstood. The aim, when prescribed thoughtfully, is not a smooth, expressionless face. It is for you to look rested and like yourself, with the deeper lines softened rather than erased.

The treatment uses small doses of a prescription-only medicine to relax specific muscles in the upper face. The areas most often treated are forehead lines, frown lines between the brows, and lines around the eyes. The effect develops over a couple of weeks and gradually fades over the following three to four months as the muscles return to their usual movement.

Dose and placement matter more than anything else. Too much in the wrong places is what creates the frozen look people worry about, and a careful, conservative approach is what keeps movement natural.

"Will I look done?" is the question I hear most often from patients considering lip fillers, and it is a fair one to ask.

The aim of a good treatment is the opposite of what people often picture. We are not trying to add volume for the sake of it, or change the shape of your face. The aim is to support what is already there: a little hydration, a softer definition, a more even balance between the top and bottom lip. If anyone notices anything afterwards, ideally it is only that you look well, not that you have had something done.

We use hyaluronic acid fillers, which are dissolvable should you ever want them reversed. Treatment itself takes around half an hour. There is some swelling for a day or two, occasionally a small bruise, and the final result settles over the following couple of weeks.

Smaller volumes tend to suit most patients, particularly if it is your first time. At the clinic we offer 0.5ml lip filler at £200 and 1ml at £300, and Dr Emily MacDonald will discuss what is realistic for your face at consultation. Not everyone is a candidate, and that is part of an honest conversation.
