This is a combination of toxin, both upper and lower face, and 1 syringe of filler to the nasolabial fold.

It is very rare that I treat the actual nasolabial fold, more often the deficit is from the medial cheek, or occasionally the pyriform fossa (next to the nose). But occasionally, it is appropriate to treat solely the nasolabial fold itself, as with this patient.

This is a very unforgiving area to treat and one I see treated poorly too often. Less is more in this area, just softening the fold, but keeping all natural expressions.

Chin filler has historically developed a poor reputation, but this is often because noticeable chin filler is usually poorly performed. As with all dermal filler treatments, the goal should be natural, balanced, and an undetectable result.

There is no one size fits all approach to chin enhancement.

Sometimes forward projection is required, as shown in the first three photos. Adding projection to a recessed chin can improve facial harmony and create a more balanced side profile.

In other cases, projection is not the issue. The concern may lie in the pre jowl area, or a patient may simply prefer a softer, rounder chin shape, as demonstrated in the next three photos.

For some patients, an overactive mentalis muscle can create a prominent chin crease. Alongside other treatments, this area can often be improved with a small amount of superficial filler to smooth the crease and enhance the overall appearance.

The key is understanding the underlying anatomy and tailoring treatment to the individual rather than applying the same approach to every patient.
