✨️VASCULAR OCCLUSION: Urgent or Emergency?

Understanding One of the Most Serious Complications of Dermal Fillers

Vascular occlusion (VO) is one of the most feared complications associated with hyaluronic acid (HA) dermal filler treatments. Although fortunately uncommon, every practitioner who performs injectable treatments must be prepared to recognise and manage it promptly.

One of the questions often asked is:

Is a vascular occlusion an emergency, or is it an urgent complication?

The answer is that, in most cases, vascular occlusion should be regarded as an urgent medical complication requiring immediate recognition and treatment, rather than an emergency requiring activation of emergency services. However, there are important exceptions where it does become a true medical emergency, particularly if there are signs of visual disturbance or central nervous system involvement.

Understanding the difference is essential for safe clinical practice.

WHAT IS A VASCULAR OCCLUSION?

A vascular occlusion occurs when blood flow through an artery is significantly reduced or completely blocked following dermal filler treatment.

This may happen through:

- Direct injection of filler into an artery.
- Compression of a blood vessel by large volumes of filler.
- Retrograde flow of filler into the arterial system followed by forward movement into smaller vessels.

When blood supply is interrupted, the tissues supplied by that artery become deprived of oxygen and nutrients (ischaemia). Without restoration of circulation, prolonged ischaemia can ultimately result in tissue necrosis.

Fortunately, when recognised early and managed appropriately, most vascular occlusions can be successfully treated before permanent tissue damage occurs.

URGENT DOES NOT ALWAYS MEAN EMERGENCY

The words "urgent" and "emergency" are often used interchangeably, but medically they have different meanings.

An urgent complication requires immediate assessment and prompt treatment to prevent deterioration.

An emergency is a life- or sight-threatening event requiring emergency medical services or immediate hospital intervention.

Most vascular occlusions are urgent because:

- The patient remains medically stable.
- Airway and breathing are unaffected.
- Treatment can usually begin immediately within the clinic.
- Early intervention significantly improves outcomes.

A vascular occlusion becomes an emergency when there is:

- Sudden loss of vision.
- Blurred vision.
- Double vision.
- Severe eye pain.
- Neurological symptoms such as weakness, altered consciousness or stroke-like features.

These situations require immediate emergency referral while appropriate management is initiated.

HOW DOES A VASCULAR OCCLUSION PROGRESS?

The progression of an occlusion often follows a predictable sequence.

STAGE 1 – Immediate Occlusion

During or shortly after injection the patient may experience:

- Sudden severe pain.
- Immediate blanching (white appearance).
- Delayed capillary refill.
- Skin becoming cool to touch.

Not every patient experiences pain, particularly in areas with reduced sensation.

STAGE 2 – Early Ischaemia

Over the next several hours the skin may develop:

- Mottled or livedoid appearance.
- Dusky or violaceous discolouration.
- Increasing tenderness.
- Sluggish circulation.

At this stage tissue is under significant stress but remains potentially salvageable.

STAGE 3 – Established Ischaemia

Without restoration of blood flow:

- Skin darkens.
- Blistering may occur.
- Increasing pain develops.
- Early tissue breakdown begins.

STAGE 4 – Tissue Necrosis

If circulation is not restored:

- Skin becomes black.
- Tissue death occurs.
- Ulceration develops.
- Healing may take weeks or months.
- Permanent scarring may result.

TIME IS CRITICAL

One of the biggest misconceptions is that treatment must occur within only a few minutes.

Current evidence suggests that earlier treatment is always preferable, but meaningful improvement may still be achieved many hours after onset.

Many expert consensus documents recommend initiating treatment as soon as vascular compromise is suspected, without waiting for symptoms to worsen.

Successful outcomes have been reported even after delayed presentation, although the likelihood of tissue injury increases as ischaemia is prolonged.

The key message is simple:

Do not delay treatment while trying to decide whether it is definitely a vascular occlusion.

MANAGEMENT OF HYALURONIC ACID VASCULAR OCCLUSION

Current UK and international expert guidance generally recommends:

- Immediate cessation of treatment.
- Thorough assessment.
- High-dose pulsed hyaluronidase administered into the affected vascular territory.
- Reassessment with repeat dosing where clinically indicated.
- Massage where appropriate.
- Warm compresses to encourage vasodilation.
- Close follow-up over the following 24–48 hours.
- Escalation where symptoms worsen or ocular involvement is suspected.

Hyaluronidase remains the recognised first-line intervention for hyaluronic acid vascular occlusion because it rapidly breaks down HA filler and can restore blood flow when administered appropriately.

CLINICAL GOVERNANCE AND ACCESS TO HYALURONIDASE

An area that deserves greater discussion is the governance surrounding emergency medicines within aesthetic practice.

In the UK, hyaluronidase is a prescription-only medicine (POM). It should only be obtained, supplied and stored in accordance with UK medicines legislation.

Importantly, practitioners should not rely on hyaluronidase prescribed in another person's name or assume they can legally hold medication intended for someone else. Prescription-only medicines should be supplied for the individual practitioner or organisation in accordance with the prescribing arrangements that apply to their practice.

Every clinic providing dermal filler treatments should have robust clinical governance and a clear pathway to access hyaluronidase without delay. This may involve appropriate arrangements with:

- A prescribing doctor.
- A prescribing dentist.
- An authorised pharmacy service.
- Another lawful prescribing model that complies with UK medicines legislation.

The overriding principle is that clinics must be able to access appropriate intervention medicines promptly and legally when managing complications.

PREPARATION SAVES TISSUE

Managing vascular occlusion begins long before a complication occurs.

PRACTITIONERS SHOULD:

- Have a written vascular occlusion protocol.
- Undertake regular complications training.
- Understand facial vascular anatomy.
- Maintain competency in recognising early signs.
- Ensure timely legal access to hyaluronidase.
- Keep appropriate emergency equipment available.
- Audit and review every complication to improve future practice.

Patients/Clients place enormous trust in practitioners. Being prepared is not optional...it is a fundamental part of safe injectable practice.

FINAL THOUGHTS

Although vascular occlusion is uncommon, it remains one of the most significant complications associated with dermal filler treatments.

Most cases represent an urgent complication requiring immediate treatment, rather than an emergency requiring hospital admission. However, any visual symptoms or neurological signs should always be treated as a true medical emergency.

Early recognition, rapid intervention, sound clinical governance and lawful access to intervention medicines remain the cornerstones of safe practice.

Ultimately, successful management depends less on luck and more on preparation.
