The Importance of Aspiration

I still see practitioners saying aspiration is pointless, it’s not accurate, and that it can’t be relied upon. Does that mean we shouldn’t do it?

I got this positive aspiration today in the cheek, through the periosteum, (on the bone) using a high-viscosity filler and a 25G needle. Did I stop injecting immediately? Absolutely. Do I usually get a positive aspiration in the cheek? No… but experiences like today are exactly why I continue to aspirate where I feel it is appropriate.

I’ve been injecting for many years, and I still prioritise safety above everything else to ensure my clients receive the highest standard of care possible.

Aspiration is not a guarantee that you’re in a safe position, and a negative aspiration should never be relied upon as proof that you’re not within a vessel. However, when you do get a positive aspiration like this, it can prevent a potentially serious complication.

Patient safety isn’t about relying on one technique—it’s about combining excellent anatomical knowledge, careful assessment, appropriate injection technique, slow injection, low pressure, continual observation, and having the skills to recognise and manage complications should they occur.

aesthetics is constantly evolving, and we should all continue learning and adapting our practice as new evidence emerges. For me, if a simple safety step has the potential to identify a vessel before I inject, I’ll continue to use it alongside every other safety measure available.
