Recently, I managed a vascular compromise following a non-surgical rhinoplasty. As an injector, these are situations you hope never happen—but you train for them because you know they can.

One of the biggest misconceptions is that complications only happen because someone has done something “wrong.” Whilst poor technique, overfilling, or inadequate assessment can increase the risk, the reality is that complications can still occur even when everything has been done correctly.

The nose is one of the highest-risk areas to treat due to its complex blood supply. This is why I always take a conservative approach. Adding too much filler, chasing perfection in one appointment, or not respecting the anatomy can all increase risk. Sometimes, less really is more.

Following the correct aftercare is equally important. Whilst it can’t prevent every complication, it supports healing and helps reduce avoidable risks.

It’s also important to understand that vascular compromise and vascular occlusion are not the same thing. A vascular compromise means blood flow has been reduced or impaired for any reason. A vascular occlusion is a blockage within a blood vessel and is a more severe type of vascular compromise. Both require prompt recognition and appropriate management.

As a Registered Nurse Prescriber, I don’t just train to achieve beautiful results, I continually train to recognise complications early, act quickly, and manage them confidently.

I share this not to create fear, but to educate. When choosing your injector, don’t just look at the results, ask about their medical background, complication training, and how they would manage an emergency if one occurred.
