Swimming Against the Current (1)
My main focus in these articles will be on regenerative medicine, my primary area of expertise, particularly exploring its facts, pros, and cons, alongside the application of stem cells.
However, it is crucial to highlight certain realities within our field. Hence, you will find me disregarding certain reports and/or studies while accusing others of being commercial reports disguised as scientific.
Seeking an impartial perspective, I turned to AI to address the bias in pharmaceutical companies and the resulting damage. The AI's response highlighted significant incidents where bias has had profound consequences:
{Bias in the pharmaceutical industry can have profound and far-reaching consequences, affecting patient safety, healthcare costs, and public trust. Here are some notable examples where bias has led to significant damage:
Selective Reporting of Clinical Trial Results:
Case of Vioxx (Rofecoxib): Merck, the manufacturer of the painkiller Vioxx, faced significant criticism and legal action after selectively reporting clinical trial data. Studies indicated Vioxx increased the risk of heart attacks and strokes, but these findings were downplayed or omitted in published reports. The drug was eventually withdrawn from the market in 2004, linked to thousands of deaths and serious cardiovascular events. This case underscored the need for transparency in clinical research.
Off-Label Marketing:
Case of Neurontin (Gabapentin): Pfizer's subsidiary, Warner-Lambert, engaged in illegal marketing by promoting Neurontin for off-label uses without sufficient evidence of efficacy and safety. The company paid over $430 million in fines, highlighting the risks of off-label marketing and potential harm to patients.
Financial Incentives and Prescribing Practices:
Case of Purdue Pharma and OxyContin: Purdue Pharma aggressively marketed OxyContin while downplaying its addictive potential, contributing to widespread misuse and addiction. The opioid crisis in the US has resulted in hundreds of thousands of overdose deaths and Purdue Pharma facing multiple lawsuits and bankruptcy.
Suppression of Negative Study Results:
Case of Avandia (Rosiglitazone): GlaxoSmithKline (GSK) faced scrutiny after suppressing data showing increased heart attack risks with Avandia. Regulatory actions followed, with GSK paying $3 billion to settle claims related to improper marketing.
Conflicts of Interest in Research:
Case of COX-2 Inhibitors: Financial ties between pharmaceutical companies and researchers influenced the development and marketing of COX-2 inhibitors. These conflicts compromised research integrity and influenced clinical guidelines, potentially impacting patient safety.
These examples illustrate the significant damage caused by bias in the pharmaceutical industry, including harm to patients, increased healthcare costs, and erosion of public trust in medicine. Addressing these biases requires stringent regulatory oversight, transparency in research and marketing practices, and ethical standards in pharmaceutical development and promotion. By learning from past mistakes, the industry can prioritize patient welfare.}
Reflecting on these examples, while shocking, prompts deeper consideration of systemic corruption. Questions arise about the role of ethics committees, auditors, and principal investigators' assistants in allowing biased practices to be published in peer-reviewed journals. The rigorous peer-review process itself raises concerns—how did these compromised studies pass through multiple stages and editors without scrutiny?
After withdrawal from the market, did those involved—publishers, professors, editors—face consequences such as job loss or legal action? It is disheartening that settlements often compensate victims monetarily without holding individuals accountable. For pharmaceutical giants, fines in the billions are insignificant compared to profits and damage caused.
Little has been done to prevent recurrence. Charitable acts and superficial gestures are insufficient to restore credibility.
As doctors and medical professionals, it is incumbent upon us not to blindly accept pharmaceutical representatives' claims. We must scrutinize and reevaluate based on our clinical experience, acting as peer reviewers in our own right.
A historical manipulation occurred in the establishment of medical evidence standards. According to GCP guidelines, randomized double-blind placebo-controlled trials (Evidence A) are regarded as the gold standard, while expert opinion (Evidence C) is marginalized. This classification allowed easy manipulation of Evidence A by altering a formula, perpetuating corruption.
Marginalizing our expert opinions laid the foundation for corruption. To be continued...

السباحة ضد التيار
بينما يردد العالم اجمع عبارة (خلونا ناكل عيش) و (اللي تكسب به العب به) أجد نفسي غير قادر أحيانا علي تطبيق هذين المبدئين وأجد نفسي مرغما علي السباحة ضد التيار.
منذ 40 عاما وبالتحديد أكتوبر 1984 عندما خطت قدمي لأول مره كليه الطب البشري جامعة القاهرة، وبعد محاضره الترحيب من أستاذنا الجليل الفاضل الأستاذ الدكتور/ هاشم فؤاد عميد الكلية آنذاك، والتي ردد خلالها أكثر من مره أنني وزملائي أفضل من أفرزت الاختبارات ليتولوا مسئوليه وأمانة الطب في جيلنا. هذا الوقت كنا نشعر بالفرحة والنشوة، ولكن بعد مرور السنين وتجد هذه الكلمات يتردد صداها في أذنك، تشعر بنوع آخر من المسئولية والهموم بسبب ما تراه يوميا من ممارسات وما ينمو لعلمك من أحداث تجعلك تشعر أنك لو استمر سكوتك عنها ستخون هذه اللحظات من الفخر التي عشتها وستخون المسئولية التي وضعت فيها والتي اختارتك الأقدار لحملها. فتجد نفسك مرغما يعلو صوتك وتسبح ضد التيار.
السباحة ضد التيار هي سلسله من البحوث العلمية والمقالات التي سأتعرض فيها للعديد من الاحداِِث الطبية والبحوث المنشورة متضمنة البحوث التي قمت بها بنفسي لنقد وتفنيد الكثير من الترهات الطبية التي تدور حولنا هذه الأيام.
لن أهديها لأحد علي قيد الحياة حتي لا يتورط معي فيما سأقول أو أنشر ولكني أهديها لأصحاب الفضل الأول والأخير بعد الله علي العبد الفقير وهم أبويا وأمي رحمهم الله.
Swimming Against the Current
While the whole world repeats the phrases "Let us eat bread" and "play the winning card" I sometimes find myself unable to apply these two principles, and I find myself forced to swim against the current.
Forty years ago, specifically in October 1984, when I set foot for the first time in the Faculty of Human Medicine at Cairo University, after the welcoming lecture by our distinguished and honorable professor, Professor Dr. Hashem Fouad, who was the Dean of the Faculty at that time. During his lecture, he repeated more than once that my colleagues and I were the best who had passed the tests to assume the responsibility and integrity of medicine in our generation. At that time, we felt joy and ecstasy. However, as the years passed and these words echoed in my ears, I began to feel a different kind of responsibility and worries. This was due to the practices I witnessed daily and the events that grew in my knowledge, making me feel that remaining silent about them would betray the moments of pride I had lived. It would betray the responsibility entrusted to me and the fate that had chosen me to carry it. Thus, I found myself forced to raise my voice and swim against the current.
Swimming Against the Current is a series of scientific research and articles in which I will present many medical events and published research, including my own, to criticize and refute much of the medical nonsense prevailing around us these days.
I will not dedicate this to anyone alive, so they do not become involved with what I will say or publish. Instead, I dedicate it to the two people I miss the most: my father and mother, may God have mercy on them.
