Dermal fillers can be used to restore an ageing face, but the goal should usually be rejuvenation and restoration rather than adding obvious volume.

The “restoration” approach

As the face ages, there is loss of bone, deep fat and soft-tissue support. This can make the cheeks flatten, temples hollow, the lower face descend and shadows develop around the mouth.

A skilled injector may therefore work in stages:

⚫️Mid-face/cheeks – small amounts can restore lost support and lift the appearance of the lower face.
⚫️Temples – carefully placed filler can soften significant hollowing.
⚫️Chin – restoring projection can improve overall facial balance.
⚫️Jawline – selected patients benefit from subtle structural support rather than creating a very sharp jaw.
⚫️Lips – restoring the border and lost volume can make an older face look fresher without creating “large” lips.
⚫️Nasolabial/marionette areas – these don’t always need to be filled directly; restoring support elsewhere may soften them naturally.

The key to avoiding an “overfilled” face

For mature faces, more filler isn’t necessarily better. I would generally favour:

restore → reassess → refine

rather than trying to correct everything in one appointment.

The injector should consider your face as a whole, including bone structure, facial width, skin laxity and how the face moves. Sometimes adding filler directly into deep folds can actually make an ageing face look heavier.

And filler can’t correct every aspect of ageing. Significant skin laxity, jowls or excess tissue may respond better to other treatments or, in some cases, surgery.
