*General Data Protection Regulation*


As you will be aware, the new rules for GDPR started today, 25th May 2018.

I am required by law to document what personal information I keep for all clients, whether they be a one off appointment or regular clients, however this information has always been required by myself for insurance purposes.

*At the time of booking an appointment I will take your name, address and a contact number.

*At your face-to-face appointment I will fill out a client consultation form. For this I will need your name, contact number, address and any medical history that you feel is relevant. This is to ensure there are no reasons why a treatment cannot go ahead.

*Some further information will be asked for, including hobbies, occupation etc which is also used to determine if the treatment is suitable.

*Client consultation forms will also include what treatment you are receiving and the date of that treatment.

*Client data will be locked in a cupboard to which only I have access.

*At your initial appointment I will ask you to fill out a 'Client Permission' form which will state how you wish to be contacted i.e for appointment reminders, special offers.
I will also ask whether I can post 'before and after' photos on my social media (no names mentioned obviously).

*I am required by law and for insurance purposes to keep this information for 7 years from the initial date, after which it will be securely destroyed, unless you are still a client.

*You have the right to see your own information and to request that it be securely destroyed. If you request the data be destroyed I will be unable to carry out future treatments.

Please be assured that I will not use your information for anything other than contacting you regarding your appointment, or to discuss matters relating to your nails and I will not pass your information on to anyone.
