Skema gerak oklusi yang ideal pada gigi anterior dirancang untuk memberikan guidance yang protektif terhadap gigi posterior dan restorasi, serta menjaga stabilitas sistem stomatognatik. Berikut penjelasannya:

1. Gerakan Protrusif (gerak maju/ke depan)
Pada gerakan protrusif, skema oklusi yang baik adalah:
Terjadi kontak simultan/serempak dan harmonis pada gigi anterior (insisivus sentral, lateral, dan dapat melibatkan kaninus). Kontak ini disebut sebagai anterior guidance. Gigi anterior berfungsi untuk:
✅️ Mendis-oklusi (memisahkan) gigi posterior sehingga mengurangi beban oklusal pada gigi posterior dan restorasi (misalnya ceramic crown/bridge)
✅️ Kontak serempak gigi anterior saat protrusif akan membagi tekanan yang terjadi saat anterior guiddance pada 6 gigi-gigi anterior sehingga tidak ada gigi yang mendapat beban berlebih yang dapat menyebabkan resiko chipping atau debonding jika ada restorasi.

2. Gerakan Laterotrusif (ke samping)
Terdapat dua skema yang dapat diterima secara klinis:
✅️ a. Canine Guidance (disclusion oleh kaninus)
Hanya gigi kaninus pada working side yang berkontak. Gigi posterior dan gigi pada balancing side tidak berkontak. Keunggulan:
Mengurangi gaya lateral pada gigi posterior
Protektif terhadap restorasi (terutama bahan keramik)

✅️ b. Group Function
Kontak serempak yang terjadi pada beberapa gigi di sisi working side (kaninus + premolar, dan molar mesiobukal). Tidak ada kontak pada balancing side.
Indikasi:
Kaninus tidak cukup kuat atau mengalami kehilangan dukungan periodontal dan untuk skema pklusi kasus rehabilitasi penuh.

⭐️Prinsip yang Harus Dihindari
Tidak boleh ada kontak pada balancing side → karena dapat menyebabkan:
✅️Premature contact
✅️Blocking
✅️Trauma oklusi
✅️Risiko fraktur atau chipping pada restorasi (terutama keramik)

Kesimpulan Klinis:
✅️Protrusif: anterior guidance → posterior harus bebas kontak
✅️Laterotrusif:
Ideal: canine guidance
Alternatif: group function
Balancing side: harus bebas kontak (non-working side disclusion)

⭐️ Skema oklusi yang tepat bukan hanya soal gigitan, tapi soal ketahanan restorasi jangka panjang.
Pada gigi asli maupun yang telah dirawat dengan crown, bridge, atau dental implant, prinsip yang digunakan adalah:
✅️ Kontak serempak (group function) pada working side untuk distribusi beban yang optimal. Jika bisa dicapai canine guide pada posisi working side maka akan lebih baik.
❌ Tidak ada kontak pada balancing side atau sisi penyeimbang, karena dapat menyebabkan premature contact atau blocking. Kontak pada balancing side ini berisiko menimbulkan chipping bahkan fraktur pada restorasi keramik—sesuatu yang tentu ingin kita hindari.

Saat gigi anterior saat gerakan protrusif: ➡️ Gigi anterior harus berkontak serempak hingga posisi edge-to-edge, sehingga melindungi gigi posterior dan restorasi dari beban berlebih.
Perencanaan oklusi yang presisi = restorasi lebih awet, fungsi lebih optimal, dan kenyamanan maksimal.

Unlike traditional full crowns, which often require extensive preparation and removal of the Bio-Rim to achieve the "ferrule effect," a bonded onlay/overlay allows for the maximal retention of this essential structural ring.

This approach fundamentally shifts the focus from the old, invasive techniques that relied on mechanical retention (like posts and full crowns) to a conservative, adhesive philosophy that prioritizes reinforcing the natural tooth structure.

These partial-coverage restorations require less preparation than a full crown, allowing for maximum preservation of the Bio-Rim and healthy coronal walls. They are used to cover and protect cusps that have been weakened by the endodontic access and prior restorations.

The Bio-Rim is a critical concept in biomimetic dentistry. It refers to the 2-3 mm of remaining healthy tooth structure, including the enamel, the dentin-enamel junction (DEJ), and superficial dentin, located coronally (above) the cemento-enamel junction (CEJ) or bone crest.

Preservation of this zone is paramount because it is the natural load-bearing area (often called the "Compression Dome"). This ring of healthy tooth structure acts to dissipate and distribute occlusal forces, protecting the remaining tooth and root from fracture.

Lithium Disilicate ceramic is chosen for its high strength, fracture toughness, and excellent aesthetics. Crucially, it is a bondable ceramic, meaning its surface can be etched (with hydrofluoric acid) and silanated to achieve a strong, durable, micromechanical bond to the resin cement then to create a "monoblock" effect, which helps to increase the tooth's resistance to fracture, unlike traditional restorations that rely on mechanical retention (like full crowns).
