In Unani medicine, Asbab-e-Sitta Zarooriya (the six essential factors) form the foundation for maintaining health and preventing diseases. Gut health is crucial in Unani philosophy, where a balanced digestive system, known as Hazm, is key to overall well-being. Let’s explore how each of the six essential factors links to gut health in Unani:
1. Hawa-e-Mohtadil (Moderate Air)
Clean, moderate air aids in detoxifying the body and supports Tabiyat (the natural healing force). Polluted or stale air can disturb the balance of Akhlat (humors) and lead to digestive problems.
2. Ghiza-e-Mo'tadil (Moderate Diet)
Food quality, quantity, and timing are central to gut health. Unani promotes a balanced diet, emphasizing mizaj (temperament) compatibility and whole foods to support the gut's health. Overeating, poor-quality foods, or incompatible diets create su-e-mizaj (imbalance), leading to indigestion, acidity, and imbalanced humors.
3. Harkat-o-Sukoon-e-Badani (Physical Activity and Rest)
Physical activity, according to Unani, aids in digestion by stimulating metabolism and supporting the liver's function in nutrient assimilation. Too much sedentary time slows metabolism and harms gut health.
4. Harkat-o-Sukoon-e-Nafsani (Mental Activity and Rest)
Emotional states impact gut health due to the brain-gut connection. Stress, anxiety, and mental strain are believed to disturb digestion, leading to conditions like constipation or diarrhea, recognized in Unani as effects of disturbed mizaj.
5. Naum-o-Yaqza (Sleep and Wakefulness)
Good sleep is essential for gut health. Unani states that sleep helps in waste removal and balances the ruh (vital spirit) to maintain mental and physical health. Poor sleep disrupts digestion and humoral balance.
6. Ihtibas-o-Istifragh (Retention and Elimination)
Retention and proper elimination (like sweating, urination, and defecation) are vital for removing waste and toxins. Imbalance here, like constipation or excessive sweating, leads to toxin accumulation, affecting gut health and the humors.
The Gut's Role in Asbab-e-Sitta Zarooriya
The gut is considered central in managing the balance of Akhlat and the natural healing force, Tabiyat. When any of these essential factors are disrupted, the digestive system is among the first to show signs of imbalance, manifesting as gastrointestinal symptoms. By addressing Asbab-e-Sitta Zarooriya, Unani aims to create an environment where the gut can function optimally, supporting overall health and balance.

In Unani medicine, each humor (blood, phlegm, yellow bile, and black bile) has distinct qualities that affect both gut and skin health. When there’s an imbalance in one or more humors, it manifests differently on the skin, often originating from disturbances in the gut. Here’s how gut health affects the skin depending on the specific humoral imbalance:
1. Blood (Dam) Imbalance❤️
Qualities: Warm and moist.
Gut Symptoms: Overeating rich, heavy foods may cause overheating in the gut, leading to inflammation.
Skin Effects: Excess blood humor may result in redness, rashes, acne, or skin conditions like rosacea. The skin may appear flushed and inflamed.
Recommendation: Cooling foods and herbs like rose water, cucumber, and barley water help balance the heat and inflammation.
2. Phlegm (Balgham) Imbalance💨
Qualities: Cold and moist.
Gut Symptoms: Weak digestion, slow metabolism, and accumulation of mucus in the digestive system.
Skin Effects: Excess phlegm results in pale, dull, or sagging skin, with possible puffiness or water retention. The skin may feel cold and clammy.
Recommendation: Warming spices like ginger, cinnamon, and black pepper are beneficial for improving digestion and circulation, helping to revitalize the skin.
3. Yellow Bile (Safra) Imbalance🔥
Qualities: Hot and dry.
Gut Symptoms: Overactivity in the liver and digestive system, leading to acid reflux, indigestion, and a burning sensation in the gut.
Skin Effects: Excessive Safra leads to dry, irritated skin, yellowish complexion, acne, and hyperpigmentation. The skin may also feel warm to the touch.
Recommendation: Cooling and hydrating foods like milk, yogurt, and sweet fruits (like melons) help soothe both the gut and the skin.
4. Black Bile (Sauda) Imbalance🕳️
Qualities: Cold and dry.
Gut Symptoms: Slow digestion, constipation, and a tendency toward bloating or heaviness in the gut.
Skin Effects: Excess black bile results in dry, rough skin with a dark or ashy complexion. It can also contribute to conditions like eczema or psoriasis.
Recommendation: Light, easy-to-digest foods, and warm soups or broths help alleviate this condition. Herbs like fennel and licorice are used to soften the gut and restore moisture
Each humoral imbalance causes distinct gut issues, which are reflected in specific skin conditions. Maintaining the balance of humors through proper diet and lifestyle is key in Unani medicine for promoting both digestive and skin health.
