The difference good oral prophylaxis can make 😎 seen over 2 weeks in a 25-y.o. female patient with initial c/o severe gum bleeding, bad breath, loss of confidence in her smile, patchy discoloration in front teeth.
Work done: complete supra- and subgingival scaling & root planing, in-office bleaching with 35% H2O2 under UV light and microabrasion and composite veneer for anterior hypoplastic area on lower front tooth (41).

Teeth whitening is a procedure to lighten the color of your teeth and remove stains. You can have it done at your dentist’s office, or do it yourself at home.
Tooth whitening is most often done using peroxide-based bleaching agents. At-home systems contain from 3% to 20% peroxide (carbamide or hydrogen peroxides). In-office systems contain from 15% to 43% peroxide.
Generally, the stronger the solution and the longer you keep it on your teeth, the whiter your teeth become. But the higher the percentage of peroxide in the whitening solution, the shorter it should stay on your teeth. Keeping it on longer will dehydrate teeth and increase tooth sensitivity.

This post is a diary entry of sorts for me:
presenting a 70-y.o. male patient with a history of avulsion of 11 while extubating him from a recent resuscitatory episode at the ER. As the patient reported to the clinic 2 weeks after the episode, there was no known way of replantation possible. The case was reviewed, radiographs taken, and laboratory investigations completed in order to proceed with extraction of all questionable teeth, thorough oral prophylaxis, and prosthodontic rehabilitation.
In dental school and beyond it, it is not uncommon to encounter situations when the patient or concerned families refuse to take your medical advice and opt for a cheaper, more convenient procedure with lesser chances of success and longevity. As a periodontist, a case like this wherein oral prophylaxis, and habit counselling are not accepted during your treatment procedure, it can be a cause for dilemma. Do you refuse treatment (my way or the highway), or do you remember that you owe all patients your best, however compromised the standard-of-care may be? Textbooks tell us that a good doctor is a good motivator, but what do you do if the initial visits are not conducive to that? Is there still hope for review appointments, or do you give up entirely?
N.B. The patient was ultimately fitted with an RPD for 11 and thorough instructions on oral hygiene maintenance, including the need for future appointments.
